Introduction: Itching is a common side effect of intrathecal morphine and epidural opioids used in cesarean delivery and other surgeries. Although many studies have been conducted on the prevention of pruritus after caesarean section, the effectiveness of various drugs in relieving pruritus is unclear.
Methods: A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. PubMed, Scopus, Web of Science, and Cochrane Central Register of Controlled Trials, and Persian SID, MagIran databases were searched with a search strategy of keywords related to pruritus, intrathecal morphine, epidural opioids, and target drugs. No time limit was applied and the search was conducted for studies from inception to August 2024. The methodological quality of the included studies was evaluated using the Jadad scoring system.
Results: 12 studies with 1430 participants were included in this systematic review. A weighted mean success rate was calculated for each treatment, showing that pentazocine had the highest success rate (96.15%), while only one study reported this, followed by nalbuphine (85.88%). Ondansetron and propofol had mixed results, with some studies finding them effective and others not. Diphenhydramine, ondansetron, and control were significantly lower, from 25.97% to 29.41%. Propofol had an average success rate of 59.76%. Most studies were of good quality.
Conclusion: Nalbuphine, pentazocine and propofol are effective in treating itching caused by intrathecal morphine and epidural opioids in caesarean section patients. Ondansetron and propofol may play a role in the treatment of pruritus, but their effectiveness is less clear.
