مروری بر تأثیر آلودگی هلیکوباکترپیلوری بر تولید مثل، بارداری و بیماری‌های زنان

چکیده

مقدمه: هلیکوباکترپیلوری علاوه بر ایجاد اختلالات معده، با کلونیزه شدن در دستگاه تناسلی، موجب برخی مشکلات زنان، نازایی و سقط می‌شود. شیوع این آلودگی در دنیا حدود 50% و در کشورهای جهان سوم بالاتر است. در مطالعات اخیر، ارتباط آلودگی باکتری و بیماری‌های زنان و بارداری، کیست تخمدان، سندرم تخمدان پلی‌کیستی، سقط و حتی ناباروری مردان مطرح شده است. مطالعه مروری حاضر با هدف بررسی تأثیر آلودگی به این باکتری بر بارداری و بیماری‌های زنانانجام شد.
روش‌کار: این مطالعه مروری با جستجوی کلیدواژه‌های Helicobacter pylori، H.Pylori، Abortion، Breast cancer، PCOS، Hyperemesis gravidarum، Anemia و Pregnancy و ترکیب احتمالی آنها با استفاده در بانک‌های اطلاعاتی Scopus، Pubmed، Iran medex, Web of Science و SID از سال 2001 تا 2017 انجام شد. سپس تمام مقالات فارسی و لاتین که دارای معیار ورود به مطالعه بودند، مورد بررسی قرار گرفتند.
یافته‌ها: تعداد 31 مقاله واجد شرایط ورود به مطالعه بودند. در اکثر این مطالعات، ارتباط Hp با ناباروری معنی‌دار بود (05/0p<)، ولی با برخی متغیرها مانند کیست تخمدان و سقط، در مطالعات مختلف تناقض وجود داشت. همچنین ارتباط معنی‌داری بین Hp با سرطان پستان و حاملگی خارج رحمی گزارش نشده بود. برخی مطالعات نقش مؤثر Hp را برکم خونی فقر آهن و تهوع شدید بارداری مطرح کرده‌اند.
نتیجه‌گیری: با توجه به نقش Hp در اتیولوژی نازایی، سقط، PCOS، آنمی و تهوع دوران بارداری، توصیه می‌شود در تمامی این موارد قبل از شروع درمان، تست سرولوژیک Hp انجام شود.

Introduction: Helicobacter pylori (Hp) infections, in addition to stomach disorders, are associated with colonization in the genital tract, causing some gynecologic problems, infertility and abortion. The prevalence of this contamination is about 50% in the world and is higher in third world countries. Recent studies have proposed the association between the infection with this bacterium and the gynecologic diseases, pregnancy problems, ovarian cysts, polycystic ovarian syndrome (PCOS / PCOS), abortion and even male infertility. This review article was performed with aim to evaluate the effect of Hp infection on pregnancy and gynecologic diseases.
Methods: This review article was performed by searching the key words including Helicobacter pylori, H. pylori, abortion, breast cancer, PCOS, hyperemesis gravidarum, anemia and pregnancy, and their possible combination using the databases of Scopus, Pubmed, Web of Science, Iran medex and SID since 2001 to 2017. Then all Persian and English articles which had the inclusion criteria were evaluated.
Results: 31 articles were eligible for inclusion in the study. In most of these studies, the significant relationship was found between Hp and infertility (P<0.05); but there was contradiction with some variables such as ovarian cyst and abortion. There was also no significant relationship between Hp and breast cancer and ectopic pregnancy. Some studies have suggested the role of Hp in iron deficiency anemia and hyperemesis gravidarum.
Conclusion: Considering the role of Hp in the etiology of infertility, abortion, PCOS, anemia and hyperemesis gravidarum, it is recommended that serologic Hp tests be performed before starting treatment in all these cases.
