بررسی ارتباط بین پلی‌مورفیسم rs2488457 ژن PTPN22 و سقط مکرر

چکیده

مقدمه: سقط مکرر به ختم بارداری بیش از دو بار اطلاق می‌شود. عوامل متعددی در ایجاد سقط مکرر دخیل هستند، وجود اختلال در عملکرد سیستم ایمنی منجر به دفع جنین می‌شود. آنزیم پروتئین تیروزین فسفاتاز بافت لنفاوی (Lyp) که توسط ژن PTPN22 کد می‌شود، نقش حیاتی در تنظیم و تعادل پاسخ‌های ایمنی دارد؛ به‌تازگی پلی‌مورفیسم rs2488457که در ناحیه پروموتر این ژن قراردارد کشف شده ‌است، محتمل است این پلی‌مورفیسم با تغییر در بیان ژن PTPN22 بالانس سیستم ایمنی را مختل نماید. از آنجایی که حفظ بالانس و تعادل سیستم ایمنی در بارداری نقش بسزایی دارد، مطالعه حاضر با هدف مقایسه‌ فراوانی پلی‌مورفیسم rs2488457 بین دو گروه زنان مبتلا به سقط مکررو زنان سالم انجام شد.
روش‌کار: این مطالعه تحلیلی از نوع مورد- شاهدی در سال 1394 بر روی 200 نفر از افراد مبتلا به سقط مکرر و 200 نفر از افراد سالم مراجعه کننده به مرکز ناباروری انجام شد. جهت بررسی ژنوتایپ افراد روش PCR-RFLP و در نهایت آزمون آماری کای‌دو جهت بررسی فراوانی پلی‌مورفیسم rs2488457 انجام شد. تجزیه و تحلیل داده‌‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 19) و آزمون آماری کای دو انجام شد. میزان p‌ کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته‌ها: فراوانی پلی‌مورفیسم rs2488457 در زنان مبتلا به سقط مکرر بیش‌تر از زنان سالم بود، اما این اختلاف از نظر آماری معنادار نبود (162/0=p).
نتیجه‌گیری: پلی‌مورفیسم rs2488457 ارتباطی با سقط مکرر ندارد.

Introduction: Recurrent Pregnancy Loss (RPL) is determined as the loss of more than two consecutive pregnancies. Several factors are involved in causing RPL. Any disorder in function of immune system leads to fetus rejection. Lymphoid-Tyrosine-Phosphatase (LYP) which is encoded by PTPN22 gene plays a pivotal role in regulation of immune responses; recently, the polymorphism of rs2488457 which is located in the promoter region of the PTPN22 gene has been discovered. This polymorphism is able to disrupt the balance of immune system by changing the PTPN22 gene expression. Since retaining the balance of immune system is of particular importance for successful pregnancy, this study was performed with aim to compare the frequency of rs2488457 polymorphism between RPL patients and normal women.
Methods: This case-control study has been conducted in 2015 on 200 recurrent pregnancy loss (RPL) patients and 200 normal individuals resorting to the infertility center of Yazd. The PCR-RFLF procedure was carried out to investigate the genotypes and the Chi-squared test was performed to inspect the rs2488457 variant frequency. The SPSS version 19 and chi-squared test were used to analyze the obtained data. p<0.05 was considered significant.
Results: The frequency of rs2488457 polymorphism was higher in RPL patients than normal women, but the difference was not statistically significant (P=0.162).
Conclusions: The rs2488457 polymorphism is not related to RPL.
