گزارش یک مورد کوریوکارسینوم متاستاتیک در بیمار یائسه

چکیده

مقدمه: بیماری ترفوبلاستیک حاملگی معمولاً در سنین باروری دیده می شود. بروز این بیماری درخانم های بعد از سن یائسگی نادر است. به همین دلیل این مورد معرفی می شود.
معرفی بیمار: خانم 60 ساله ای است که با شکایت خونریزی و توده واژینال به بیمارستان قائم (عج) دانشگاه علوم پزشکی مشهد مراجعه نموده و بعد از نمونه برداری از توده مزبور، بر اساس آسیب شناسی متاستاز کوریوکارسینوم تشخیص داده شد. بررسی جهت تعیین و ارزیابی وسعت درگیری بیماری انجام شد و در نهایت تشخیص کوریوکارسینوم مرحله IIIc داده شد. لذا بیمار تحت شیمی درمانی با رژیم EMA-CO ( اتوپوزاید، متوترکسات، اکتینومایسین، سیکلوفسفاماید، وین کرسیتین) قرار گرفته و بعد از 7 دوره درمان بهبودی کامل یافت.
نتیجه‌گیری: هدف از گزارش این مورد توجه بیش تر به امکان وجود بیماری های تروفوبلاستیک حاملگی در سنین مختلف و با تابلوهای متفاوت است.

Introduction:Gestational trophoblastic neoplasia (GTN) usually occurs during women’s reproductiveages and it is very unusual in postmenopausal women. The aim of this study is to present a case of GTN in a postmenopausal woman.
 
Case Report: A 60-year-old woman with history of vaginal bleeding and a mass lesion was admitted to Ghaem hospital affiliated to Mashhad university of medical sciences. She afterwards underwent surgical biopsy of vaginal mass and histological evaluation revealed metastatic choriocharcinoma. Investigation for extention of metastases performed and the final diagnosis was a stage IIIc choricaricnoma. Patient received EMA-CO regimen chemotherapy (Etoposide, Methotrexate, Actinomycine, Cyclophosphamide, Vincristin) and after seven courses of chemotherapy she experienced a complete relief.
 
Conclusion: Physicians should not ignore the possibility of GTN in different ages with a variety of presentations.
