بررسی تأثیر استنشاق اکسیژن بر میزان خونریزی پس از زایمان

چکیده

مقدمه: خونریزی پس از زایمان از علل اصلی مرگ مادران در سراسر جهان و مسئول تقریباً 30% از موارد مرگ مستقیم مادری می باشد. با توجه به اهمیت کنترل خونریزی پس از زایمان در زندگی مادر و کودک، مطالعه حاضر با هدف بررسی تأثیر استنشاق اکسیژن بر خونریزی پس از زایمان انجام شد.
روش کار: این مطالعه کارآزمایی بالینی در سال 92-1391 بر روی 120 زن باردار مراجعه کننده به زایشگاه بیمارستان شهید مصطفی خمینی شهر ایلام جهت انجام زایمان طبیعی انجام شد. افراد به روش نمونه گیری آسان انتخاب و به طور تصادفی در دو گروه مداخله و کنترل قرار گرفتند. در هر دو گروه جهت اداره مرحله سوم زایمان از یک لیتر سرم رینگر و 20 واحد اکسی توسین استفاده شد. در گروه مداخله علاوه بر اداره معمول، از 8 لیتر در دقیقه اکسیژن از طریق ماسک صورت به طور مداوم تا 2 ساعت پس از زایمان استفاده شد. میزان خونریزی با توزین شان و گازها و پدهای مورد استفاده محاسبه و یک گرم افزایش وزن معادل یک سی سی خون در نظر گرفته شد. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 16) و آزمون های کای اسکوئر و تی انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد.
یافته ها: نتایج حاصل از آزمون تی نشان داد که میانگین خونریزی در پایان دو ساعت اول بعد از مداخله در گروه مداخله 46±5.149 سی سی و در گروه کنترل 97±16.256 سی سی بود که اختلاف معنی داری بین دو گروه وجود داشت (006/0>p).
نتیجه گیری: استنشاق اکسیژن در طی مرحله سوم لیبر و دو ساعت اول بعد از زایمان باعث کاهش معنی دار در میزان خونریزی پس از زایمان می شود.

Introduction: Postpartum hemorrhage is of the major causes of maternal mortality worldwide, and is responsible for approximately 30% of maternal direct death. Given the importance of management of postpartum bleeding for mother and child, this study was performed with aim to evaluate the effect of Oxygen inhalation on postpartum hemorrhage.
Methods: This clinical trial study was performed on 120 pregnant women referred to the Maternity of Shahid Mustafa Khomeini Hospital of Ilam from 2012 to 2013, for normal vaginal delivery. The subjects were selected with convenience sampling and were randomly assigned to two groups. In two group, for Management of the third stage of labor, one liter of ringer and 20 units of oxytocin were used. In the intervention group, in addition to the routine administration, 8 liters of oxygen via face mask was continuously used until 2 hours after delivery.  Blood loss was determined by regular weighting of the buttocks and gases and pads, and 1 gram increase in weight was considered to be equivalent to 1cc blood). Data was analyzed by SPSS software (version 16) using T-test and Chi-square test. PResults: The results of t-test indicated that the mean of bleeding was 256.16±97 cc in control group and 149.5±46 cc in intervention group at the end of first two hour after intervention that the difference between two groups was significant (p<0.006).
Conclusion: Oxygen inhalation during the third stage of labor and the first two hours after delivery significantly decreases postpartum hemorrhage.
