مقایسه عوامل جنینی و معیار های سونوگرافی آندومتر در دوره های درمانی موفق و ناموفق کمک باروری

چکیده

مقدمه: لانه گزینی موفق جنین در آندومتر در برنامه های کمک باروری حاصل تداخل مناسب و بموقع عوامل جنینی و رحمی است. مطالعه حاضر به منظور مقایسه پارامتر های جنینی و شاخص های سونوگرافی آندومتر در سیکل های موفق و ناموفق درمانی کمک باروری صورت گرفت.
 
روش کار: این مطالعه توصیفی در سال 1384 در پژوهشکده رویان تهران انجام شده است. 108 بیماری که به علل مختلف نازایی در سیکلهای لقاح آزمایشگاهی (IVF) و تلقیح داخل سیتوپلاسمی اسپرم(ICSI) قرار گرفته بودند وارد مطالعه شدند. سونوگرافی واژینال داپلر رنگی در روز انتقال جنین صورت گرفت. وقوع حاملگی که با رویت ساک حاملگی در سونوگرافی مشخص شده برای هر بیمار پرسشنامه ای مشتمل بر خصوصیات بیمار و سیکل درمانی، پارامتر های سونوگرافی آندومتر و تعداد و کیفیت جنین های انتقال یافته تنظیم شد. اطلاعات جمع آوری شده با استفاده از برنامه آماری SPSS و امور توصیفی پردازش شد.
 
نتایج: حاملگی در 36 نفر(3/33%) بوقوع پیوست. میانگین سنی، مدت و علت نازایی، میانگین FSH و استرادیول پایه و استرادیول روز تزریق HCG[1]و میزان داروی مصرفی در دو گروه حامله و غیر حامله با هم تفاوت آماری معنی داری نداشت.ضخامت و الگوی آندومتر، شاخص ضربان پذیری[1] و مقاومت شریان رحمی نیز در دو گروه تفاوتی نداشت در حالیکه تعداد و کیفیت جنین های انتقال یافته در گروه حامله به طور معنی داری بیشتر و بالاتر از گروه غیر حامله بود. 
نتیجه گیری: در شرایط مشابه از نظر خصوصیات بیمار و سیکل درمانی، تعداد و کیفیت جنین های انتقال یافته به رحم دربرنامه های IVF و ICSI از اهمیت کلیدی در وقوع حاملگی برخوردارند در حالی که معیار های سونوگرافیک مرسوم آندومتر شاخص قابل توجهی در پیش بینی موفقیت سیکل درمانی نیستند.

Comparison of Ultrasonographic Parameters of Endometrium and Embryonic Variables in Successful and Unsuccessful Assisted Reproductive Techniques

Introduction: Successful implantation during assissted reproductive technique
cycles depend on many factors including embryonic and uterine variables. The aim of
this study was to investigate the difference between embryonic and endometrial
ultrasonographic parameters in patients who had undergone In Vitro Fertilization
(IVF) and Intracytoplasmic (ICSI) in successful and failed cycles.
Methods: In this descriptive study 108 patients aged<38 with basal serum FSH
level<12 and no more than two failed attempts of ART cycles, whose uterin were
confirmed by ultrasound to be morphologically normal, were studied. Ultrasound
evaluation was performed by a single expert on the day of embryo transfer.
Ultrasonographic parameters of endometrium and embryonic variables were analyzed
based on the occurrence of pregnancy.
Results: 33.3% of patients conceived. No significant difference was found between
pregnant and non-pregnant group regarding demographic, hormonal and cycle
characteristics and ultrasonographic parameters. On the contrary, embryonic factors
differed significantly between the two groups.
Conclusion: While embryonic factors significantly influence successful
implantation, there is no relationship between ultrasonographic parameters of
endometrium on the day of embryo and implantation.
