ارزیابی شیوع درد شانه پس از جراحی سزارین با بیهوشی عمومی- مقاله کوتاه

چکیده

مقدمه: درد شانه پس از سزارین، از جمله شکایات بیماران است. هدف از اجرای این مطالعه، تخمین شیوع درد شانه و ارتباط آن با عمل جراحی سزارین انتخابی در بیماران با بیهوشی عمومی بوده است.
 
روش کار: مطالعه حاضر، مطالعه توصیفی مقطعی است که جامعه آماری پژوهش در بیمارستان امام رضا (ع) و بیمارستان امام زمان (عج) مشهد 930 نفر بودند و از این تعداد 332 نفر به‌صورت تصادفی انتخاب شدند. پرسشنامه‌ای توسط پژوهشگران شامل سن، وجود یا عدم وجود درد، محل درد (شانه راست یا چپ)، شدت درد (خفیف، متوسط و شدید) و محل نصب کاتتر وریدی بوده، طراحی شد و برای هر بیمار با روش مصاحبه، پر شد. تجزیه و تحلیل آماری با آزمونهای آماری تی مستقل، کای‌دو و آزمون دقیق فیشر و با استفاده از نرم‌افزار SPSS (نسخه 14) انجام شد.
 
یافته‌ها: یافته‌ها نشان داد که شیوع درد شانه در عمل جراحی سزارین انتخابی، 131 نفر (45/39%) بوده و ارتباط معنی‌داری بین میزان درد (‌بی‌دردی، خفیف، متوسط و شدید‌) و سمت شانه وجود داشت. بیشترین شیوع درد شانه در شانه راست به میزان متوسط بود (05/0p<) و همچنین ارتباط معنی‌داری بین شیوع درد شانه پس از سزارین و جراح آن و تعداد عملهای سزارین انجام شده توسط آن جراح وجود داشت (05/0p<).
 
نتیجه گیری: این مطالعه مشخص می‌کند که درد شانه به دنبال سزارین در بیماران با بیهوشی عمومی، عارضه قابل توجهی است و این درد با شدت متوسط و در شانه راست بیشتر ایجاد می شود.

چکیده [English]

Introduction: Shoulder pain is one of the patient complaints after cesarean section. The aim of this study was to estimate the prevalence of shoulder pain and its association with elective cesarean section in patients with general anesthesia.
 
Methods: An observational cross-sectional study was conducted with statistical society of 930 patients in Emam Reza and Emam Zaman hospitals in Mashhad using random sampling of 332 participants. To assess the prevalence of shoulder pain in elective cesarean with general anesthesia, a questionnaire was designed that consisted of age, existence or non existence of pain, severity of pain (severe, moderate or mild) occurrence of pain in right or left shoulder, and location of intravenous catheter. Statistical analysis was done using SPSS version 14, by independent t-test, chi-square and fisher exact tests.
 
Results: The prevalence of shoulder pain in elective cesarean section was found to be 131 patients(39.45%). There was a positive correlation between pain range (without pain, mild, moderate, severe) and side of painful shoulder. The most prevalent rate of shoulder pain was in right shoulder with moderate severity (p<0.05). There was a positive correlation between the prevalence of shoulder pain after cesarean section and each gynecologist and the number of her cesarean section (p<0.05).
 
Conclusion: The present study highlights the significance of shoulder pain in elective cesarean with general anesthesia that is more in right shoulder with moderate severity.
