مقایسه تاثیر تیپینگ و تمرین درمانی بر ناتوانی عملکردی و لوردوز کمری در سه ماهه دوم بارداری

چکیده

مقدمه: تغییرات وضعیت بدنی زنان باردار از عوامل بروز لوردوز کمری در ستون مهره ها و ناتوانی ناشی از آن است. یکی از راه های کاهش آن، انجام حرکات ورزشی است. نواربندی (تیپینگ) نیز از روش های درمانی جدید می باشد. مطالعه حاضر با هدف مقایسه تأثیر تیپینگ و تمرین بر ناتوانی عملکردی و لوردوز کمری در سه ماهه دوم بارداری انجام شد.
روش کار: این مطالعه کارآزمایی بالینی در سال 1393 بر روی 90 زن باردار 24-16 هفته بارداری که جهت دریافت خدمات به مراکز بهداشتی درمانی منتخب شهر مشهد مراجعه کردند، انجام شد. این مطالعه به صورت سه گروهه (تمرین، تیپینگ، کنترل) و به مدت شش هفته صورت گرفت. در گروه تمرین تمرینات کششی و تقویتی یک روز در میان، روزی دو بار به مدت 15 دقیقه انجام شد و در گروه تیپینگ، تیپ بر روی عضلات دو طرف ستون فقرات به موازات T12- L5 و یک نوار بر روی قسمت تحتانی تعبیه شد. جهت سنجش ناتوانی از مقیاس ناتوانی عملکردی اسوستری و جهت اندازه گیری قوس کمر از خط کش منعطف استفاده شد. تحلیل و تحلیل داده ها با استفاده از آزمون آنالیز واریانس دو طرفه با اندازه های تکراری، آنالیز واریانس یک طرفه و تی زوجی انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد.
یافته ها: در گروه تمرین و تیپینگ، ناتوانی عملکردی پس از شش هفته کاهش معنی‌داری داشت (001/0>p)، اما تغییرات در گروه کنترل معنی‌دار نبود (072/0=p). در گروه تمرین، لوردوز بعد از شش هفته کاهش معنی داری داشت (002/0>p) ولی این  تغییرات در گروه تیپینگ (776/0=p) و کنترل (586/0=p) معنی‌دار نبود.
نتیجه گیری: تمرین و تیپینگ به یک اندازه ناتوانی عملکردی را در زنان باردار کاهش می‌دهد و از تمرینات می‌توان در کاهش لوردوز کمری استفاده شود.

چکیده [English]

Introduction: changes in the physical condition of pregnant women is one off the factors of lumbar lordosis in the spine and its related disabilities. Exercise is one of the methods to reduce it. Taping is of new therapies. This study was performed with aim to compare the effect of taping and exercise on functional disability and lumbar lordosis in the second trimester of pregnancy.
Methods: This randomized clinical trial was performed on 90 pregnant women with gestational age of 16-24 weeks referred to health centers of Mashhad to receive services in 2014. This study was conducted in three groups (exercises, taping and control) for six weeks. In exercise group, stretching and strengthening exercises were performed every other day, twice a day for 15 minutes, and in taping group, the tape was inserted on two strip muscles on either side of the spine parallel to the T12- L5 and a tape on the lower back. The Oswestry functional disability questionnaire was used to assess the disability and flexible ruler was used to measure lumbar lordosis. Data analysis was performed by two-way analysis of variance with repeated measures, ANOVA and paired t-test. PResults: In exercises and taping groups, functional disability after six weeks was significantly decreased (P<0.001), but the changes were not significant in the control group (P=0.072). In exercise group, lordosis after six weeks was significantly reduced (P<0.002), but these changes were not significant in taping (P=0.776) and control (P=0.586) groups.
Conclusions: Exercise and taping as equal reduce the functional disability in pregnant women and the exercises can be used to reduce the lumbar lordosis.
