چکیده

مقدمه: در بیماران نابارور مبتلا به سندرم پلی‌کیستیک که عدم پاسخ به درمان‌های تحریک تخمک‌گذاری دارند، لقاح آزمایشگاهی (IVF) گزینه مناسبی است. مطالعه حاضر با هدف مقایسه نتایج بارداری در بیماران سندرم تخمدان پلی‌کیستیک تحت درمان IVF که تحت پروتکل GnRH آگونیست بلندمدت یا آنتاگونیست GnRH قرار می‌گیرند، انجام شد.
روش‌کار: این مطالعه کارآزمایی بالینی تصادفی در سال 1393 بر روی 96 بیمار نابارور مبتلا به سندرم تخمدان پلی‌کیستیک کاندید لقاح آزمایشگاهی (IVf‌) مراجعه‌کننده به مرکز میلاد انجام شد. بیماران به‌طور تصادفی به 2 گروه آگونیست GnRH بلندمدت و آنتاگونیست GnRH قرار گرفتند و به هر دو گروه قرص ضد بارداری خوراکی به‌عنوان پیش درمان و FSH نوترکیب جهت تحریک تخمدان تجویز شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 20) و آزمون‌های آماری کی دو، دقیق فیشر، و من‌ویتنی انجام شد. میزان p کمتر از 05/0 معنی‌دار در نظر گرفته شد.
یافته‌ها: دو گروه آگونیست GnRH و آنتاگونیست GnRH از نظر نتایج بارداری نظیر حاملگی شیمیایی (544/0=p)، حاملگی بالینی (6/0=p)، سقط (1=p) و چندقلویی (668/0=p) تفاوت آماری معناداری نداشتند. شیوع سندرم تحریک بیش از حد تخمدان در گروه آنتاگونیست نسبت به آگونیست کمتر بود که از نظر آماری معنادار بود (0001/0=p). دوز گنادوتروپین (344/0=p) و تعداد روزهای تحریک با گنادوتروپین (236/0=p) بین دو گروه از نظر آماری معنادار نبود.
نتیجه‌گیری: آنتاگونیست GnRh می‌تواند در مبتلایان به سندرم تخمدان پلی‌کیستیک که کاندید IVF هستند، مورد استفاده قرار گیرد و استفاده از رژیم آنتاگونیست GnRh در مقایسه با آگونیست GnRH بر روی نتایج بارداری تفاوتی ندارد و فقط عارضه درمان ناباروری را کاهش می‌دهد.

چکیده [English]

Introduction: IVF is a good option for Polycystic ovary syndrome patients who not respond to ovulation induction treatment. This study was performed with aim to compare pregnancy outcomes in PCOS patients undergoing IVF with long GnRH agonist versus flexible GnRH antagonist regimens.
Methods: This randomized clinical trial study was performed on 96 infertile PCOS women candidates for IVF who referred to Milad Infertility Center in 2014. The patients were divided into 2 groups: long GnRH agonist and GnRH antagonist. Oral contraceptive pills were prescribed as pretreatment and recombinant FSH was given for ovarian induction. Data were analyzed by SPSS software (version 20) and Chi-square, Exact Fisher, and Mann-Whitney tests. P<0.05 was considered statistically significant.
Results: Two groups of GnRH agonist and GnRH antagonist were not significantly different in terms of pregnancy outcomes like chemical pregnancy (p=0.544), clinical pregnancy (p=0.6), abortion (p=1), and multiple pregnancy (p=0.688). The prevalence of Ovarian hyperstimulation syndrome was less in antagonist group versus agonist group that had statistically significant difference (p=0.0001). Dose of gonadotropins (p=0.344) and the number of days of gonadotropins consumption (p=0.236) were not significantly different between two groups.
Conclusion: GnRH antagonist can be used in PCOS patient's candidates for IVF. The use of GnRH antagonist is not different in pregnancy outcome compared to GnRH agonist and only decreases the complications of infertility treatment.
