گزارش یک مورد بارداری محل انسزیون درمان شده با متوتروکسات

چکیده

مقدمه: حاملگی نابجا در اسکار سزارین، نادر بوده و در صورت عدم درمان زودهنگام، با عوارض بالایی همراه است. در بین روش­های درمانی، درمان انتظاری با بالاترین میزان شکست و عارضه همراه است. تزریق متوتروکسات سیستمیک در صورتی‌که علائم حیاتی پایدار بوده و در صورت سطح تیتراژ BHCGکمتر از 5000، سن حاملگی کمتر از 8 هفته و عدم وجود قلب جنین، با موفقیت بالایی همراه است و نسبت به روش­های جراحی باعث حفظ باروری می­گردد. در این مقاله، یک مورد حاملگی نابجا در اسکار سزارین که با یک دوز متوتروکسات تحت درمان قرار گرفت و بهبود یافت، گزارش می‌شود.
معرفی بیمار: بیمار خانم 33 ساله با حاملگی سوم، دارای دو فرزند حاصل سزارین با سن بارداری 7 هفته و 4 روز بود که با درد شکم، لکه‌بینی و سونوگرافی ترانس واژینال مبنی بر یک ساک حاملگی در محل اسکار سزارین و تیتراژ BHCG 3900 بستری شد و با توجه به شرایط بالینی، بیمار کاندید درمان طبی شد و یک دوز متوتروکسات عضلانی دریافت کرد و پس از آن تیتراژ BHCG کاهش پیشرونده داشت و بیمار با حال عمومی خوب مرخص شد.
نتیجه‌گیری: درمان طبی بارداری نابجا در اسکار سزارین علاوه بر اثربخشی و کاهش عوارض، باعث حفظ باروری در بیمار می‌شود.

Introduction: Cesarean scar ectopic pregnancy is rare and is associated with serious complications if not treated early. Among treatment methods, the expectant treatment is associated with high risk for failure and complications. In patients with stable beta-human chorionic gonadotropin (βhCG) titers < 5000, gestational age < 8 weeks, and absence of fetal heart tones, administration of systemic methotrexate is associated with high success rate compared to surgical methods, and it is more likely to preserve fertility. In this article, we present a case of cesarean scar ectopic pregnancy treated with a single dose of methotrexate.
Case presentation: The patient was a 33-year-old woman (gravidity: 3, parity: 2, gestation: 7 weeks and 4 days) with history of two cesarean sections. She was admitted with abdominal pain, spotting, transvaginal ultrasonography based on a gestational sac implanted in cesarean scar, and βhCG titers of 3900. According to the patient's clinical condition, she was candidate for medical treatment and received one dose of methotrexate, then βhCG titers progressively reduced, and the patient was discharged in good general condition.
Conclusion: Medical treatment of cesarean scar ectopic pregnancy, in addition to efficacy and reduction of complications, can preserve fertility in the patient.
