تخمین وزن جنین به‌وسیله ارتفاع رحم با مدل‌های رگرسیون و درخت تصمیم در بیمارستان امام خمینی شهر اهواز در زمستان 1393

چکیده

مقدمه: برای کنترل بهتر دوران بارداری و تصمیم‌گیری مناسب برای ختم حاملگی به‌ویژه در موارد پرخطر، دانستن وزن تخمینی جنین حائز اهمیت است. مطالعه حاضر با هدف بررسی وزن جنین با اندازه‌گیری ارتفاع رحم، جهت دسترسی به یک روش آسان، ارزان و کاربردی برای نقاط محروم انجام گرفت.
روش‌کار: این مطالعه اپیدمیولوژیک تحلیلی بر روی 220 زن باردار دارای شرایط خاص که در سال 1393 جهت زایمان به بیمارستان امام خمینی شهر اهواز مراجعه کرده بودند، انجام شد. ارتفاع رحم با استفاده از متر نواری توسط کارشناس مامایی اندازه‌گیری و وزن تخمینی جنین با استفاده از فرمول بن‌ جانسون به‌دست آمد. وزن نوزاد بلافاصله پس از تولد با ترازوی استاندارد سکا اندازه‌گیری و سایر عوامل مؤثر بر وزن جنین مورد بررسی قرار گرفت. سپس داده‌ها با استفاده از رگرسیون و درخت تصمیم، مورد بررسی قرار گرفتند. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماریSPSS (نسخه 17) و آزمون‌های آماری کای اسکوئر و رگرسیون آنوا انجام شد. میزان p‌ کمتر از 05/0 معنا‌دار در نظر گرفته شد.
یافته‌ها:میانگین وزن واقعی نوزادان 52/3244 گرم و میانگین وزن تخمینی نوزادان 18/3304 گرم بود. بین وزن نوزاد هنگام تولد با ارتفاع رحم، ارتباط مستقیم و معناداری وجود داشت (01/0=p). فرمول حاصل از رگرسیون به صورت "تعداد زایمان × 94/48 + ارتفاع رحم (سانتی‌متر) × 6/80 = W" به‌دست آمد. همچنین با استفاده از مدل‌سازی با درخت‌ تصمیم بر اساس ارتفاع رحم، تخمین مناسبی برای محاسبه وزن جنین حاصل گردید.
نتیجه‌گیری:اندازه‌گیری ارتفاع رحم به عنوان معیار مناسبی جهت پیش‌بینی وزن جنین می‌تواند مورد استفاده قرار بگیرد و با استفاده از این ویژگی می‌توان برخی مشکلات مربوط به کمبود وزن نوزادان و رشد غیر‌طبیعی جنین را قبل از زایمان پیش‌بینی کرد.

چکیده [English]

Introduction: To better control of pregnancy period and appropriate decision for termination of pregnancy, especially in high-risk cases, estimation of fetal weight is important. This study was performed with aim to evaluate fetal weight by fundal height measurement to access an easy, inexpensive and practical method in deprived areas.
Methods: This analytical epidemiological study was performed on 220 pregnant women with special conditions who had referred Ahwaz Imam Khomeini Hospital for delivery in 2014. The uterine height was measured using a tape meter by a midwife and estimated fetal weight was obtained using Ben Johnson formula. The birth weight was measured immediately after birth with standard Seca scale and other factors affecting birth weight were studied. Then, data were studied by Regression and Decision Tree. Data were analyzed by SPSS software (version 17) and Chi-square and ANOVA regression tests. P<0.05 was considered significant.
Results: Mean of real birth weight was 3244.52 gr and mean of estimated birth weight was 3304.18 gr. There was a direct and significant relationship between mean of birth weight and fundal height (P=0.01). The formula obtained from the regression was "W= 80.6 ×Fundal Height (cm) + 48.94 × number of delivery". Moreover, the well-estimated birth weight was obtained using the modeling with the decision tree based on the fundal height.
Conclusion: Measuring the fundal height can be used as an appropriate criteria to predict the fetal weight. This feature can be used to predict some of the problems related to low birth weight and abnormal fetal growth before delivery.
