بررسی علل، روشهای درمانی و عواقب حاملگی در بیماران با خونریزیهای غیرطبیعی در سه ماهه اول بارداری
مقدمه: بیش از 25% از زنان باردار در سه ماهه اول و اوایل سه ماهه دوم بارداری دچار خونریزی میشوند که در نیمی از این بیماران سقط اتفاق میافتد. علل و روشهای مختلفی برای تشخیص و درمان خونریزیهای دوران بارداری وجود دارند. با توجه به شیوع نسبتاً بالای خونریزیهای سه ماهه اول بارداری و روشهای تشخیصی و درمانی توصیه شده، مطالعه حاضر با هدف بررسی علل، شیوههای تشخیصی و درمانی و عواقب حاملگی این بیماران در بخش زنان بیمارستان امیرالمومنین سمنان انجام شد.
روش کار: این مطالعه مقطعی و آیندهنگر در سال 93-1392 بر روی 200 بیمار بستری در بخش زنان بیمارستان امیرالمومنین (ع) سمنان با خونریزیهای سه ماهه اول بارداری انجام شد. اطلاعات مربوط به بیماران با حاملگی اثبات شده شامل علل خونریزی، روشهای تشخیصی و درمانی، عواقب و پیامدهای آنها بررسی و اطلاعات در پرسشنامههای محقق ساخته ثبت شد. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 16) و روش های آمار توصیفی انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد.
یافته ها: علل خونریزیهای غیر طبیعی در بیماران در 2 مورد (1%) وجود تخمک خالی، در 52 مورد (26%) سقط ناقص، در 62 مورد (31%) سقط فراموش شده، در 24 مورد (12%) سقط کامل، در 54 مورد ( 27%) تهدید به سقط، در 2 مورد (1%) حاملگی خارج رحمی، در 1 مورد (5/0%) حاملگی مولار، در 2 مورد (1%) حاملگی شیمیایی یا سقط زودرس و در 1 مورد (5/0%) پولیپ سرویکس بود. سقط درمانی در 56 نفر (27/48%) از بیماران به روش مدیکال و جراحی، در 40 نفر (48/34%) فقط با درمان دارویی و در 20 مورد (24/17%) فقط با درمان جراحی به تنهایی انجام شد. از 54 مورد تهدید به سقط، 20 مورد (05/37%) منجر به سقط، 10 مورد (5/18%) منجر به زایمان زودرس و 22 مورد (4/47%) منجر به زایمان ترم شد. سن بارداری در تمام بیماران زیر 9 هفته بود و در 70% موارد با سابقه سقط، مجدداً سقط اتفاق افتاد.
نتیجهگیری:علل خونریزیهای غیر طبیعی منجر به بستری در سه ماهه اول بارداری به ترتیب شامل سقط اجتناب ناپذیر، تهدید به سقط، حاملگی خارج رحمی، حاملگی مولار و پولیپ سرویکس بود. شایع ترین علل منجر به بستری سقط اجتناب ناپذیر و شایع ترین روش درمانی دارویی+ جراحی بود. در این مطالعه نیمی از موارد تهدید به سقط با زایمان ترم، 37% با سقط و 18% با زایمان زودرس همراه، حاملگی زیر 9 هفته و سابقه سقط با خطر خونریزی بیشتر و سقط همراه بود.

بررسی تأثیر اسانس شوید و مفنامیک اسید بر پس درد زایمانی
مقدمه: پس درد، درد ناشی از انقباضات متناوب رحمی بعد از خروج جفت و پرده ها است. گاهی اوقات جهت برطرف کردن آن نیاز به مسکن میباشد. اما به دلیل اثرات مضر داروها تمایل به استفاده از داروهای گیاهی بیشتر شده است. مطالعه حاضر با هدف بررسی مقایسه اثر اسانس شوید و مفنامیک اسید بر پس درد زایمانی انجام شد. روش کار: این مطالعه کارآزمایی بالینی در سال 1394 بر روی 108 زن چندزایی که به روش طبیعی زایمان کرده و دارای پس درد زایمان بودند، انجام شد. افراد به طور تصادفی در دو گروه 54 نفری اسانس شوید و مفنامیک اسید قرار گرفتند. شدت پس درد 2 ساعت پس از زایمان با استفاده از خط کش درد اندازه گیری شد و در صورت داشتن نمره 1/3 و بالاتر مورد مطالعه قرار گرفتند و هر 6 ساعت تا 4 بار در صورت ادامه یافتن درد، داروی اسانس شوید (5/1 میلی گرم اسانس به ازای هر کیلوگرم از وزن بدن تقسیم بر 20) و یا مفنامیک اسید (250 میلی گرم) دریافت میکردند. تجزیه و تحلیل دادهها با استفاده از نرم افزار آماری SPSS (نسخه 22) و آزمون های تی تست، کای دو و اندازهگیری مکرر انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد. یافته ها: میانگین شدت درد پس از زایمان بعد از مداخله در دو گروه کاهش یافت، ولی دو گروه اختلاف آماری معنی داری با هم نداشتند (05/0<p). نتیجه گیری: اسانس شوید مانند مفنامیک اسید باعث کاهش پس درد زایمانی میشود، لذا میتواند به عنوان یک داروی ضد درد گیاهی برای مادرانی که پس درد زایمانی دارند مورد استفاده قرار گیرد.

بررسی مطالعات مرتبط با دیسمنوره اولیه در بُعد ابزار سنجش درد قاعدگی
مقدمه: دیسمنوره اولیه از شایعترین شکایات زنان است. بررسی بیمار و درمان موفق وی نیازمند سنجش میزان درد است. با توجه به شیوع بالا و متفاوت دیسمنوره اولیه در مطالعات و شناخت مقبولترین ابزارهای بررسی درد دیسمنوره و به بحث گذاشتن مزایا و معایب این ابزارها، مطالعه حاضر با هدف بررسی مطالعات مرتبط با دیسمنوره در بُعد ابزار سنجش درد قاعدگی انجام شد. روش کار: این مطالعه مروری با هدف بررسی مطالعات مرتبط با دیسمنوره در بُعد ابزار سنجش درد قاعدگیانجام شد.بدین منظور پایگاههای اطلاعاتی Pubmed Medline، Embase، Cochran و Google Scholar جهت یافتن منابع مرتبط در فاصله سالهای 2003 تا 2013 و با کلید واژههایی نظیر dysmenorrhea، scale و ... جستجو شدند. محتوای مقالات با دقت و به تکرار مطالعه شد و با تمرکز بر ابزار بررسی درد از بُعد کیفی و کمی مورد تجزیه و تحلیل قرار گرفت. یافته ها: در جستجوی به عمل آمده در مجموع تعداد 259 مقاله به دست آمد. از این تعداد 248 مقاله از ابزارهای ذهنی و 11 مقاله از ابزارهای عینی دیسمنوره استفاده کردند. پرکاربردترین ابزارهای ذهنی VASو معیار چندبُعدی گفتاری دیسمنوره و پرکاربردترین ابزار عینی دیسمنوره بررسی سطح پروستاگلاندینها بود. به طور کلی از 28 ابزار متفاوت برای بررسی درد دیسمنوره استفاده شده بود. در مقالات فارسی 9 ابزار به کار برده شده بود. به علاوه 199 مقاله (6/78%) از یک ابزار، 48 مقاله (18%) از دو ابزار، 10 مقاله (8/2%) از سه ابزار و 2 مقاله (4/0%) از چهار ابزار همزمان استفاده کرده بودند. نتیجه گیری: به طور کلی ابزارهای متنوع و زیادی برای ارزیابی درد قاعدگی اولیه در دسترس هستند، ولی هنوز یک استاندارد طلایی وجود ندارد. اما VAS و معیار چندبُعدی گفتاری دیسمنوره نسبت به بقیه ابزارها بیشتر استفاده میشوند. لازم است ابزار مناسب با توجه به هدف پژوهشگر از تحقیق، هزینه انجام مطالعه، مشخصات جامعه مورد بررسی و ویژگیهای ابزار انتخاب شود.
