رابطه پارگی زودرس کیسه آب قبل از ترم با بیماری های پریودنتال
مقدمه: عوامل بسیاری مادران را مستعد پارگی زودرس پرده های جنینی و تولد نوزادان نارس می کند. از آنجایی که بیماری پریودنتال به عنوان یک فاکتور مستعد کننده پارگی زودرس کیسه آب مطرح شده است، مطالعه حاضر با هدف بررسی ارتباط بین پارگی زودرس کیسه آب با شاخص های پریودنتال انجام شد. روش کار: این مطالعه مورد- شاهدی در سال 91-1390 بر روی 100 مادر باردار مراجعه کننده به بیمارستان امام رضا (ع) و قائم (عج) مشهد انجام شد. بیماران در دو گروه 50 نفره مورد (پارگی زودرس کیسه آب قبل از ترم) و شاهد (پارگی زودرس کیسه آب در ترم) مورد ارزیابی قرار گرفتند. تمام این افراد از نظر سیستمیک سالم بودند. در معاینه انجام شده شاخص های خونریزی از لثه، شدت و وسعت بیماری پریودنتال، پلاک دندانی و عمق پروبینگ با استفاده از پروب پریودنتال ویلیامز مورد بررسی قرار گرفت. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 5/11) و آزمون های تست دقیق فیشر، کای دو، تی تست، من ویتنی و تی دانشجویی انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد. یافته ها: در این مطالعه بین میانگین شاخص خونریزی، شاخص شدت، وسعت بیماری پریودنتال، میانگین عمق پروپینگ و شاخص پلاک دندانی در دو گروه مورد و شاهد اختلاف معناداری مشاهده شد (05/0p<). نتیجه گیری: بیماری های پریودنتال می تواند به عنوان یک عامل خطر مستقل برای پارگی زودرس و قبل از ترم کیسه آب باشد.

اثر بخشی آموزش مدیریت استرس به شیوه شناختی رفتاری بر هیجانات منفی و کیفیت زندگی زنان مبتلا به سرطان پستان
مقدمه: بیشتر مبتلایان به سرطان، یک دوره فشار روانی را تجربه می کنند. یکی از بیماریهاییکه با سلامت روانی ارتباط زیادی دارد، سرطان است. شواهد متعددی وجود دارد که نشان می دهد درمان استرس تأثیر مثبتی بر نتایج درمان سرطان دارد. مطالعه حاضر با هدف تعیین اثربخشیآموزش مدیریت استرس به شیوه شناختی رفتاری بر استرس، اضطراب، افسردگی و کیفیت زندگی زنان مبتلا به سرطان پستان انجام شد. روش کار: این مطالعه کاربردی در سال 1391 بر روی 32 نفر از زنان تازه تشخیص داده شده در مرحله یک و دو سرطان سینه مراجعه کننده به مرکز درمانی امام رضا (ع) شهر مشهد انجام شد. افراد به طور تصادفی در دو گروه آزمون و کنترل قرار گرفتند.گروه آزمون تحت 10 هفته آموزش مدیریت استرس به شیوه شناختی رفتاری توسط یک روانشناس با تجربه در این حوزه قرار گرفتند. پرسشنامه افسردگی، اضطراب و استرس DASSو پرسشنامه کیفیت زندگی 36SF به عنوان ابزار در پیشآزمون و پسآزمون استفاده شدند. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 11) و آزمون های تی مستقل، آزمون f لوین و تحلیل کواریانس انجام شد. میزان p کمتر از 05/0 معنادار در نظر گرفته شد. یافتهها: تغییرات استرس و اضطراب در پسآزمون بین گروه های مداخله و کنترل معنادار بود (01/0=p). در مورد افسردگی، تفاوت معناداری بین دو گروه پس از مداخله مشاهده نشد (به ترتیب60/0=p، 50/0=p)، در گروه مداخله پس از انجام مداخله تفاوت معناداری بین نمرات پیش آزمون و پس آزمون مقیاس کیفیت زندگی در نمرات کیفیت زندگی مشاهده شد (02/0=p). نتیجهگیری: آموزش مدیریت استرس به شیوه شناختی رفتاری بر استرس، اضطراب و کیفیت زندگی در زنان مبتلا به سرطان مؤثر است.

مقدمه: مطالعات انجام شده در زمینه یائسگی در طول زمان از نظر رویکردها و دیدگاه های فکری محققان بسیار متفاوت بوده اند. آشنایی با رویکردهای گوناگون تحقیقاتی می تواند در طراحی برنامه های مراقبتی مبتنی بر شواهد کمک کننده باشد، لذا مطالعه حاضر با هدف بررسی سیر تحول در رویکردهای تحقیقاتی در موضوعات مرتبط با یائسگی انجام شد. روش کار: در این مطالعه مروری 60 مقاله مرتبط با یائسگی تا 12 اردیبهشت ماه 1394 از بانک های اطلاعاتی Science Direct، Scopus، PubMed، Google Scholar، Magiranو SID با کلید واژه های "یائسگی"، "دوران گذر"، "یائسه"، "رویکرد زیست پزشکی"، "رویکرد روانی- اجتماعی" و "رویکرد طبیعت گرایانه" جستجو شدند. این مقالات از نظر رویکرد زیربنایی تحقیق بررسی و بر اساس تابعیت از سه رویکرد زیست پزشکی، روانی- اجتماعی و طبیعت گرایانه در جداول مقایسه ای تنظیم و مورد قیاس موضوعی قرار گرفتند. یافته ها: رویکرد زیست پزشکی یائسگی را معادل با شرایط کاهش هورمون استروژن و بیماری دانسته و به دنبال درمان آن است. رویکرد روانی- اجتماعی به دنبال تفسیرهای دیگری به جز کاهش استروژن نظیر سندرم آشیانه خالی برای توضیح علائم یائسگی می باشد و رویکرد طبیعت گرایانه به دنبال کشف تجربیات، احساسات و نیازهای عمیق زنان از زبان خودشان هستند. نتیجه گیری: آشنایی با هر سه رویکرد مطالعاتی و استفاده از نتایج آنها با ایجاد یک نگاه جامع نگر به یائسگی می تواند راهنمای سیاست گذاران و برنامه ریزان در جهت طراحی مداخلات کل نگر و تحقق اهداف پزشکی مبتنی بر شواهد در حیطه یائسگی باشد.
