بررسی فراوانی پروتئین P53 و BCL2 و میزان آنژیوژنز در سرطان آندومتر و ارتباط آن با شاخص های کلینیکو پاتولوژیک
مقدمه: P53 در بیش از 50% تومورهای توپر جهش یافته است و در سرطان آندومتر که شایع ترین بدخیمی دستگاه تناسلی زنان می باشد، در 40-30% موارد تغییر کرده است و تغییر مزبور با نوع سلول سروز پاپیلری و مرحله بالا و پیش آگهی بد همراه است. همچنین میزان آنژیوژنز تومور و بروز بیش از حد BCL2 در پیش آگهی بیماری دارای اهمیت می باشد، بنابراین مطالعه حاضر با هدف بررسی بروز پروتئین P53 و BCL2 و میزان آنژیوژنز در سرطان آندومتر و ارتباط آن با شاخص های کلینیکو پاتولوژیک و عوامل مؤثر بر پیش آگهی بیماری انجام شد. روش کار: در این مطالعه طولی گذشته نگر طی سال های 93-1384، پرونده های 30 مورد از بیماران مبتلا به سرطان آندومتر در سه بیمارستان امام رضا (ع)، قائم (عج) و امید جمع آوری شد. نمونه های آسیب شناسی از نظر مارکرهای P53 و Bcl2 و آنژیوژنز مورد رنگ آمیزی ایمنوهیستوشیمی قرار گرفتند و ارتباط این مارکرها با عوامل مؤثر بر پیش آگهی بیماری و عود بیماری مورد بررسی قرار گرفت. تجزیه و تحلیل داده ها با استفاده از آزمون های آماری تی تست، کای اسکوئر و Log rank انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد. یافته ها: بروز P53 در 9 مورد از 10 بیمار (90%) با سرطان غیر آندومتریوئید مثبت بود و تمام بیماران از نظر Bcl2 منفی بودند و 8 مورد از 10 بیمار (80%) با نوع غیر آندومتریوئید و 6 مورد از 20 بیمار با انواع آندومتریوئید (30%) از نظر آنژویوژنز شدید بودند. بین نوع بافت شناسی تومور و درجه و مرحله تومور و نیز بقاء بیماری ارتباط معنی داری وجود داشت (002/0=p). نتیجه گیری: با توجه به ارتباط بین مارکرهای ژنتیکی با مرحله و درجه بیماری و نیز بقاء بیماران، بهتر است هنگام تشخیص پاتولوژی سرطان آندومتر، بلوک مربوط از نظر این مارکرها بررسی شود.

بررسی ارتباط بین ریز مغذی های مادران فعال و غیر فعال با رشد نوزادان
مقدمه: کمبود ریز مغذی ها، یکی از مشکلات اساسی سلامت جامعه در بسیاری از کشورهای پیشرفته است. در این میان، کودکان و مادران باردار در معرض خطر بیشتری قرار دارند. مطالعه حاضر با هدف بررسی ارتباط بین ریزمغذی های روی، آهن، سلنیوم و اسید فولیک مادران باردار فعال و غیرفعال با رشد جسمانی نوزادانشان انجام شد. روش کار: این مطالعه هم گروهی آینده نگر در سال92-1391 بر روی کلیه زنان باردار 35-16 ساله مراجعه کننده جهت کنترل بارداری به بیمارستان های دولتی در مشهد انجام شد. 5 سی سی خون از افراد گرفته و نمونه ها به آزمایشگاه فوق تخصصی ارسال شدند. میزان فعالیت بدنی مادران نیز توسط پرسشنامه میزان فعالیت شارکی سنجیده شد. پس از به دنیا آمدن نوزادان، با مراجعه به پرونده پزشکی مادران میزان قد، وزن و دور سر نوزادان آنها به دست آمد. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 5/11) انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد. یافتهها: بین وزن نوزادان با میزان روی در مادران غیر فعال همبستگی معنی داری وجود داشت (02/0=p، 52/0=r)، اما بین قد و دور سر نوزادان مادران فعال و غیر فعال با ریزمغذی های آهن و اسیدفولیک و سلنیوم ارتباطی یافت نشد (05/0<p). شاخصهای رشدی نوزادان مادران فعال از نوزادان مادران غیر فعال بیشتر بود، اگرچه این تفاوت ها از نظر آماری معنی دار نبود (1/0<p). نتیجهگیری: ریز مغذی روی میتواند بر افزایش وزن نوزادان مؤثر باشد. همچنین مادران فعال، نوزادان بهتری از نظر شاخص رشدی دارند.

مرور کلی بر پاپیلوما ویروس ها به عنوان اصلی ترین عامل سرطان دهانه رحم
مقدمه: سرطان دهانه رحم مرتبط با پاپیلوما ویروس ها به عنوان شایع ترین سرطان زنان به صورت یک معضل بهداشتی در سراسر دنیا خصوصاً در کشورهای در حال توسعه مطرح می باشد. ژنوتیپ های مختلف پاپیلوما ویروس های انسانی خصوصاً ژنوتیپ های پرخطر به عنوان عامل اصلی این سرطان و دیگر سرطان های انسانی نظیر: دستگاه تناسلی، پوست، سر و گردن و غیره مطرح هستند. مطالعه حاضر با هدف افزایش آگاهی در مورد ساختار، بیماری زایی و تشخیص پاپیلوما ویروس ها انجام شد. روش کار: مرور مستندات در مطالعه مروری حاضر با جستجو در پایگاه های علمی معتبر خارجی و داخلی و با استفاده از کلمات کلیدی مرتبط صورت گرفت. در این مطالعه سعی شده است با استفاده از مقالات منتشر شده در این پایگاه های علمی به بررسی نقش ژنوتیپ های مختلف پاپیلوما ویروس ها در ایجاد عفونت های مختلف از جمله سرطان دهانه رحم و روش های تشخیصی موجود در جهت ژنوتایپینگ این دسته از ویروس ها پرداخته شود. یافته ها: افزایش دانش روز افزون درباره نقش مهم و اساسی پاپیلوما ویروس ها در سرطان زایی آنها منجر به بهبود روش های نوین تشخیصی مولکولی همراه با بیومارکرهای مولکولی در ژنوتایپینگ پاپیلوما ویروس ها خواهد شد. نتیجه گیری: گسترش روز افزون عفونت های تناسلی، بیماری های مقاربتی و سرطان های ناشی از میکروارگانیسم ها به خصوص پاپیلوما ویروس، همکاری جوامع جهانی در انجام مطالعات جامع تر به خصوص در کشورهای در حال توسعه را می طلبد. آموزش همگانی، توسعه غربالگری کلاسیک همراه با روش های نوین تشخیصی، بهترین ابزار در تعیین خط مشی در استراتژی های درمانی و پیشگیری کننده چون واکسن بر علیه سرطان های مرتبط با پاپیلوما ویروس ها در کشورهای در حال توسعه خواهد بود.
