بررسی اثر آروماتراپی بر روی کیفیت خواب دوران پریمنوپوز و منوپوز: مرور سیستماتیک و متاآنالیز
مقدمه: یکی از شایعترین مشکلات زنان یائسه، اختلال خواب میباشد. این اختلال میتواند با علائم خفیف تا شدید و ناتوان کننده در افراد مختلف بروز نماید. یکی از رویکردهای مورد استفاده جهت رفع اختلال خواب، استفاده از طب مکمل از جمله آروماتراپی است. مطالعه مرور سیستماتیک و متاآنالیز حاضر با هدف ارزیابی و جمعبندی نتایج کارآزماییهای بالینی انجام شده در مورد تأثیر آروماتراپی بر اختلالات خواب زنان پره و پست منوپوز انجام شد. روشکار: جهت انجام این مطالعه، منابع الکترونیکی لاتین مانند PubMed، Scopus، ISI Web of Science، Cochrane Library و پایگاههای اطلاعاتی فارسی مانند SID و Magiran بهصورت سیستماتیک بدون محدودیت زمانی تا تاریخ 10 فوریه 2018 مورد جستجو قرار گرفتند. جهت ارزیابی کیفیت مقالات از مقیاس جداد استفاده شد. ساعت خواب و یا کیفیت خواب گزارش شده در مقاله به عنوان پیامد اصلی مطالعه در نظر گرفته شد. یافتهها: در 4 مطالعه یافت شده از اسانس اسطوخودوس به تنهایی و یا همراه سایر روشها و رایحهها جهت آروماتراپی استفاده شده بود. نتایج متاآنالیز انجام شده بر روی سه مطالعهای که کیفیت خواب را گزارش کرده بودند، نشان داد که مقدار اختلاف میانگین استاندارد شده بین دو گروه مداخله و کنترل (347/2-329/0 CI: 95%) 338/1 بود که این اختلاف نشان دهنده بهبودی کیفیت خواب در گروه مداخله در مقایسه با گروه کنترل بود (009/0=p). در مطالعهای که ساعت خواب نمونهها گزارش شده بود، اختلاف معنیداری بین دو گروه مداخله و کنترل یافت نشد (373/0=p). نتیجهگیری: رایحه درمانی با اسانس اسطوخودوس بهصورت تنها یا در ترکیب با سایر روشها میتواند اثرات سودمندی بر بهبود کیفیت خواب زنان پره و پست منوپوز داشته باشد.

شیوع سندرم پاهای بیقرار در زنان باردار در آسیا: مطالعه مرور سیستماتیک و متاآنالیز
مقدمه: سندرم پاهای بیقرار نوعی اختلال نورولوژیکی است که با بروز اختلال خواب منجر به بیماریهای قلبی عروقی، فشارخون بارداری، افسردگی و افزایش مرگومیر مادر و جنین میشود. مطالعات مختلف شیوعهای متفاوتی را گزارش کردهاند. مطالعه مرور سیستماتیک و متاآنالیز حاضر با هدف برآورد شیوع سندرم پاهای بیقرار در زنان باردار در آسیا انجام شد.
روشکار: در این مطالعه مروری با جستجو در پایگاههای اطلاعاتی ملی و بینالمللی SID، Magiran،Google Scholar، IranMedex، Science Direct، PubMed و Scopus تعداد 16 مقاله مشاهدهای دارای متن کامل مربوط به سالهای 2003 تا ژانویه 2018 استخراج شدند. جهت انجام این مطالعه متاآنالیز، از مدل اثرات تصادفی استفاده گردید و ناهمگنی مطالعات با شاخص I2 بررسی شد. آنالیز دادهها با نرمافزار STATA (نسخه 12) انجام شد.
یافتهها: شیوع سندرم پاهای بیقرار در زنان باردار آسیایی در 16 مقاله بررسیشده با حجم نمونه 49342 نفر، 5/18% (فاصله اطمینان 95%: 38/23-75/13%) بود. شیوع سندرم پاهای بیقرار بر اساس مصاحبه 04/14% (فاصله اطمینان 95%: 05/16-02/12%) و بر اساس پرسشنامه 75/22% (فاصله اطمینان 95%: 01/32-49/13%) بود. بین شیوع سندرم پاهای بیقرار در بیماران با سال انتشار مقالات (939/0=p)، حجم نمونه (161/0=p) و میانگین سن نمونهها (105/0=p) ارتباط معناداری مشاهده نشد.
نتیجهگیری: سندرم پاهای بیقرار در زنان باردار آسیایی از شیوع بالایی برخوردار است. به دلیل پیامدهای مادری و جنینی، بررسی علل و عوامل دقیق ابتلاء به این بیماری در این گروه پرخطر ضروری بهنظر میرسد.

مروری بر راهکارهای حفظ باروری بیماران سرطانی در درمانهای رایج
مقدمه: سالانه افراد زیادی در جهان به انواع سرطانها مبتلا میشوند. امروزه با مراقبتهای ویژه و روشهای درمانی متنوع چند گزینهای و ترکیبی، طول عمر در بسیاری از افراد سرطانی افزایش یافته است. افزایش پتانسیل بقاء با درمان سرطان، اغلب با اثرات جانبی از جمله تأثیر بر باروری و کیفیت آن در بیماران جوان همراه است. اقدامات ضروری جهت حفظ پتانسیل باروری در طی درمان سرطان از اهمیت ویژهای برخوردار است. مطالعه مروری حاضر با هدف بررسی جدیدترین روشهای حفظ باروری در حین درمانهای سرطان انجام شد. روشکار: در این مطالعه مروری برای جستجوی مقالات از سه پایگاه اطلاعاتی Google Scholar، Scopus و PubMed و کلید واژههای infertility cancer، fertility cancer، infertility chemotherapy و fertility chemotherapy استفاده شد. مقالاتی که هدف نگارش این مقاله را در بر نداشتند، از مراحل مطالعه حذف گردید. یافتهها: راهکارهای متنوعی جهت حفظ باروری بیماران سرطانی پس از درمانهای متداول وجود دارد. انجماد و تهیه بانک اسپرم، تخمک و جنین، انجماد بافت بیضه و تخمدان، استفاده از عوامل حفظ باروری و انواع سلولهای بنیادی، از مهمترین روشهای استاندارد حفظ باروری در بیماران سرطانی تحت درمان هستند. نتیجهگیری: امروزه قبل از هر اقدام درمانی سرطان، لازم است بیماران مشاوره مناسب مرتبط با خطرات باروری و حمایتهای مرتبط جهت گزینههای حفظ باروری را دریافت کنند.

بررسی وضعیت شیوع ناهنجاریهای مادرزادی در ایران: مطالعه مرور سیستماتیک و متاآنالیز
مقدمه: ناهنجاریهای مادرزادی از مهمترین علل بستری در بیمارستان و معلولیت و مرگ نوزادان به شمار میروند و هزینه بستری و اقدامات درمانی این کودکان، بار سنگینی را به سیستم بهداشتی و خانوادههای آنها وارد میکند. از این رو مطالعه حاضر با هدف بررسی وضعیت ناهنجاریهای مادرزادی در ایران به روش مرور سیستماتیک و متاآنالیز انجام شد.
روشکار: در این مطالعه مروری جهت یافتن مطالعات مرتبط، پایگاههای اطلاعاتی SID،Iranmedex ، IranDoc،Magiran ، Scopus و Pubmed و همچنین موتور جستجوی Google Scholarبا استفاده از کلید واژههای شیوع، ایران، ناهنجاریهای مادرزادی و معادل انگلیسی آنها در مجلات فارسی و انگلیسی زبان در فاصله سالهای 1365 تا اردیبهشت 1397 جستجو شدند. دادهها با روش متاآنالیز و با استفاده از مدل اثرات تصادفی مورد تجزیه و تحلیل قرار گرفت و ناهمگنی مطالعات با استفاده از شاخص I2محاسبه شد.
یافتهها: از مجموع 2306 مطالعه بعد از اعمال معیارهای ورود و خروج، در نهایت 35 مطالعه از فاصله سالهای 1986 تا 2017 وارد متاآنالیز شدند. نتایج این مطالعه متاآنالیز نشان داد شیوع کلی ناهنجاری مادرزادی در ایران 6/2% میباشد. شیوع ناهنجاری در پسران 8/2% و در دختران 2% میباشد. شایعترین ناهنجاری مادرزادی گزارش شده در متولدین ناهنجاری اسکلتی (1/29%) و سپس ناهنجاری سیستم ادراری تناسلی (2/23%) میباشد.
نتیجهگیری: نتایج مطالعه حاضر حاکی از میزان بالای ناهنجاری مادرزادی در ایران میباشد، لذا مطالعات بیشتری جهت شناخت بیشتر این مسأله و برنامهریزی در جهت پیشگیری از بروز ناهنجاری مادرزادی توصیه میشود.

کاربرد طب فشاری گوش در حوزه زنان و مامایی: مرور سیستماتیک کارآزماییهای بالینی
مقدمه: طب فشاری گوش، یک تکنیک غیرتهاجمی و ایمن ذکر شده و یکی از مواردی است که پزشکان، ماماها، پرستاران و حتی خود بیماران میتوانند از آن استفاده کنند. مطالعات متعددی در خصوص طب فشاری گوش در درمان بیماریها و مشکلات مختلف زنان انجام شده است، لذا مطالعه حاضر با هدف مروری بر کاربردهای طب فشاری گوش در زنان و مامایی انجام شد.
روشکار: در این مطالعه مروری مقالات نمایه شده در پایگاههای اطلاعاتی Pumped، Google scholar، Magiran، Irandoc، Medline، Scoupus، Web of Science در بازه زمانی 2006-2018 با استفاده از کلید واژههای مبتنی بر MeSHشامل طب گوش یا طب فشاری گوش (OR Auriculotherapy Acupressure) با استفاده از AND با کلید واژههای دیسمنوره، یائسگی، تهوع و استفراغ، یبوست، کمردرد، زایمان، بارداری و ناباروری به صورت جداگانه مورد بررسی قرار گرفتند. معیارهای ورود به مطالعه شامل درج کلمات مورد جستجو در بخش عنوان و کلید واژههای مقالات در کارآزماییهای بالینی انتشار یافته به زبان فارسی و انگلیسی بود. مقالاتی که طب فشاری و سوزنی را به صورت ترکیبی استفاده کرده بودند، از مطالعه خارج شدند. جهت ارزیابی کیفیت مقالات از ابزار جداد استفاده شد.
یافتهها: در این مطالعه 12 کارآزمایی بالینی بررسی شد. از 3 مقاله مرتبط با یائسگی، 1 مقاله تأثیر معنادار با طب فشاری گوش را نشان داد؛ از 2 مقاله مرتبط با دیسمنوره، 1 مقاله تأثیر معنادار طب فشاری گوش در کاهش درد و عواطف منفی را نشان داد؛ از 2 مقاله مرتبط با درد زایمان، 1 مقاله تأثیر معنادار طب فشاری گوش در کاهش درد را گزارش کرد و از 2 مقاله مرتبط با اضطراب بعد از سزارین، 1 مقاله طب فشاری گوش را در کاهش اضطراب معنادار گزارش کردند. مقاله مرتبط با یبوست نیز بهبود معنادار را در صورت استفاده از طب فشاری گوش نشان داد و در مورد تهوع و ناباروری نتایج معنیدار نبود.
نتیجهگیری: طب فشاری گوش میتواند در بهبود مشکلات زنان کمک کننده باشد و به عنوان یک روش مکمل و جایگزین دارودرمانی پیشنهاد شود، اما با توجه به کم بودن تعداد مطالعات انجام شده در این زمینه، گزارش نتایج ضد و نقیض و همچنین محدودیت برخی مطالعات، انجام پژوهشهای بیشتر با رفع محدودیتها و اشکالات مطالعات قبل، پیش از بکارگیری در بالین پیشنهاد میشود.

درمانهای طبیعی الیگومنوره بر اساس طب ایرانی
مقدمه: الیگومنوره، یکی از شایعترین اختلالات قاعدگی است که به صورت افزایش فواصل قاعدگی تعریف میشود. این اختلال رو به افزایش به دلیل عوارضی مانند خونریزی رحمی غیرطبیعی، ناباروری، هیرسوتیسم و عواقب روانی- اجتماعی متعاقب آنها باعث کاهش کیفیت زندگی زنان سنین باروری میشود. با توجه به عوارض داروهای هورمونی که درمان رایج این اختلال میباشد، ضرورت استفاده از روشهای درمانی مکمل مطرح میشود، لذا مطالعه مروری حاضر با هدف بررسی راهکارهای درمانی طب ایرانی انجام شد. روشکار: در این مطالعه مروری، منابع اصلی طب ایرانی با استفاده از واژگان مترادف الیگومنوره مانند احتباس طمث، حبس حیض، کاهش و قطع قاعدگی بررسی شد. برای یافتن مطالعات جدید نیز پایگاههای اطلاعاتی مانند Google Scholar، PubMed و ISI با کلیدواژگان Oligomenorrhea، Amenorrhea، PCOS، Persian Medicine و Traditional Medicine جستجو و نتایج حاصل مقایسه شدند. یافتهها: در منابع طب ایرانی، الیگومنوره با تعبیر "احتباس طمث" آمده است. رویکردهای درمانی احتباس طمث، مانند سایر درمانها در طب ایرانی، در گام اول شامل اصلاح سبک زندگی، اصلاح الگوی غذایی، تنظیم خواب و بیداری، ورزش و استحمام میباشد. گام دوم شامل: مصرف گیاهان دارویی قاعدهآور است که اغلب فیتواستروژن محسوب میشوند. این گیاهان به اشکال مختلف (قرص، جوشانده، دمنوش، شیاف واژینال، دوش واژینال، ضماد، آبزن و ...) تجویز میشوند. در گام سوم، اعمال یداوی مانند حجامت و فصد نیز بهعنوان یک روش درمانی توصیه شده است. نتیجهگیری: با توجه به اثبات اثربخشی تعدادی از این درمانها در مطالعات جدید مانند گیاه پنج انگشت، پونه و رازیانه انجام مطالعات بالینی بیشتر بر مبنای درمانهای طب ایرانی در کنار طب رایج، جهت درمان الیگومنوره توصیه میشود.

مروری بر تأثیر آلودگی هلیکوباکترپیلوری بر تولید مثل، بارداری و بیماریهای زنان
مقدمه: هلیکوباکترپیلوری علاوه بر ایجاد اختلالات معده، با کلونیزه شدن در دستگاه تناسلی، موجب برخی مشکلات زنان، نازایی و سقط میشود. شیوع این آلودگی در دنیا حدود 50% و در کشورهای جهان سوم بالاتر است. در مطالعات اخیر، ارتباط آلودگی باکتری و بیماریهای زنان و بارداری، کیست تخمدان، سندرم تخمدان پلیکیستی، سقط و حتی ناباروری مردان مطرح شده است. مطالعه مروری حاضر با هدف بررسی تأثیر آلودگی به این باکتری بر بارداری و بیماریهای زنانانجام شد.
روشکار: این مطالعه مروری با جستجوی کلیدواژههای Helicobacter pylori، H.Pylori، Abortion، Breast cancer، PCOS، Hyperemesis gravidarum، Anemia و Pregnancy و ترکیب احتمالی آنها با استفاده در بانکهای اطلاعاتی Scopus، Pubmed، Iran medex, Web of Science و SID از سال 2001 تا 2017 انجام شد. سپس تمام مقالات فارسی و لاتین که دارای معیار ورود به مطالعه بودند، مورد بررسی قرار گرفتند.
یافتهها: تعداد 31 مقاله واجد شرایط ورود به مطالعه بودند. در اکثر این مطالعات، ارتباط Hp با ناباروری معنیدار بود (05/0p<)، ولی با برخی متغیرها مانند کیست تخمدان و سقط، در مطالعات مختلف تناقض وجود داشت. همچنین ارتباط معنیداری بین Hp با سرطان پستان و حاملگی خارج رحمی گزارش نشده بود. برخی مطالعات نقش مؤثر Hp را برکم خونی فقر آهن و تهوع شدید بارداری مطرح کردهاند.
نتیجهگیری: با توجه به نقش Hp در اتیولوژی نازایی، سقط، PCOS، آنمی و تهوع دوران بارداری، توصیه میشود در تمامی این موارد قبل از شروع درمان، تست سرولوژیک Hp انجام شود.

ارزیابی نقش مارکرهای سرمی و سونوگرافی داپلر شریان رحمی در پیشگویی پرهاکلامپسی در سه ماهه دوم بارداری: یک مطالعه مرور سیستماتیک
مقدمه: پرهاکلامپسی، شایعترین عارضه طبی در بارداری است. با توجه به پاتوفیزیولوژی و اتیولوژی پیچیده پرهاکلامپسی، طیف وسیعی از مارکرها بررسی شدهاند که علاوه بر ارزش پیشگویی، شناسایی این عوامل ممکن است مکانیسم روشنی برای پاتوژنز پرهاکلامپسی فراهم کند. مطالعه مروری حاضر با هدف ارزیابی نقش چهار مارکر سرمی و سونوگرافی داپلر شریان رحمی در پیشگویی پرهاکلامپسی در سه ماهه دوم بارداری انجام شد.
روشکار: در این مطالعه مروری جهت یافتن مطالعات مرتبط، در بازه زمانی 2000 تا 2017 از پایگاههای اطلاعاتی SID، Weily online library،Elsevier ، Scopus، Pubmed و همچنین موتور جستجوی Google Scholar استفاده شد. برای دستیابی به تمامی مقالات به زبان انگلیسی و فارسی مورد نظر از کلید واژههای فارسی شامل: BHCG، اینهیبین A، اکتیوین A، آلفا فتوپروتئین، مارکرهای سرمی، سونوگرافی داپلر شریان رحمی، سه ماهه دوم و پیشگویی پرهاکلامپسی و کلید واژههای انگلیسی شامل: serum markers،Uterine artery Doppler ultrasonography، Preeclampsia prediction، Second trimester، AFP، β-HCG، Activin A، Inhibin A با همه ترکیبات احتمالی این کلمات استفاده شد.
یافتهها: نتایج مطالعات مختلف بیانگر این مسئله بودند که هیچ یک مارکرهای مورد مطالعه شامل اکتیوین A و اینهیبین A، β-HCG و AFP به تنهایی قدرت پیشگویی پرهاکلامپسی را با حساسیت و ویژگی بالا فراهم نمیکنند، اما سونوگرافی داپلر شریان رحمی در سه ماهه دوم بارداری را میتوان به عنوان یک تست غربالگری برای پیشگویی پرهاکلامپسی در زنان پرخطر بهکار برد.
نتیجهگیری: مهمترین استراتژیهای پیشگویی پرهاکلامپسی شامل: مشخصات فردی، سونوگرافی شریان داپلر و بیومارکرها میباشند و هیچیک از این مارکرها به تنهایی قدرت پیشگویی پرهاکلامپسی را با حساسیت و ویژگی بالا فراهم نمیکنند.

بررسی تأثیر زنجبیل بر دیسمنوره اولیه: مروری سیستماتیک بر کارآزماییهای بالینی و مطالعات نیمهتجربی انجام شده در جهان
مقدمه: دیسمنوره، اختلال ژنیکولوژیک شایعی میباشد که در هر سیکل قاعدگی، باعث بروز مشکلات رفتاری و اختلال در روند زندگی روزمره یک زن میگردد. زنجبیل در درمان دیسمنوره ایفای نقش مینماید، از این رو مطالعه مروری حاضر با هدف جمعآوری و بررسی کارآزماییهای بالینی در رابطه با تأثیر زنجبیل خوراکی و موضعی بر دیسمنوره انجام شد. روشکار: در این مطالعه مروری، کارآزماییهای بالینی و مطالعات نیمهتجربی تا دسامبر 2017 به زبان فارسی و انگلیسی در پایگاههای فارسی IranMedex، Magiran، IranDoct، IRCT و پایگاههای انگلیسی PubMed، Scopus،Science direct ، Embase و Cochrane و با کلید واژههای فارسی دیسمنوره، قاعدگی دردناک، زنجبیل و انگلیسیDysmenorrhea ، Menstrual pain، Menstrual cramp، Painful menstruation، Zingiber، Ginger، Shokyo، Gingiber officinale و Zingier بدون اعمال محدودیت زمانی مورد بررسی قرار گرفتند و مقالات دارای معیار ورود، وارد مطالعه شده و مورد آنالیز کیفی قرار گرفتند. یافتهها: پس از بررسی 2101 مقاله، 12 کارآزمایی بالینی که بر اساس معیار جداد نمره 3 و بالاتر و 3 مطالعه نیمه تجربی بر اساس معیار EPHPP با امتیاز 00/3-51/1 مورد بررسی قرار گرفتند. در اکثر مقالات به خاصیت ضدالتهابی و مهار سیکلواکسیژناز و به دنبال آن کاهش لوکوترین و پروستاگلاندین در هر دو فرم خوراکی و موضعی زنجبیل اشاره شده بود که در تسکین درد دیسمنوره مؤثر بودند. به دلیل ناهمگونی مطالعات، امکان انجام متاآنالیز وجود نداشت. نتیجهگیری: با توجه به نتایج بهدست آمده، شواهد قابل اعتباری مبنی بر تأثیر مثبت زنجبیل خوراکی و موضعی بر کاهش درد و التهاب بهخصوص در سه روز اول قاعدگی وجود دارد.

بررسی تأثیر تحریک ویبروآکوستیک و نور بر تستهای ارزیابی سلامت جنین: یک مطالعه مروری
مقدمه: یکی از اجزای اساسی مراقبتهای دوران بارداری، ارزیابی سلامت جنین میباشد که آزمونهای متفاوتی در این زمینه وجود دارد. به تازگی در برخی مطالعات تحریک با ویبروآکوستیک و نور هالوژن نیز به عنوان تست ارزیابی سلامت جنین بررسی شده است؛ مطالعه مروری حاضر با هدف جمعبندی مطالعات سالهای اخیر در زمینه تأثیر تحریک با ویبروآکوستیک و نور هالوژن بر جنین انجام شد.
روشکار: در این مطالعه مروری به منظور دستیابی به مستندات داخلی و خارجی مرتبط، جستجوی گسترده از پایگاههای اطلاعاتی داخلی magirin، Irandoc،SID ، Google scholarو پایگاههای اطلاعاتیScopus ، PubMed و science directصورت گرفت. جستجوی مقالات با استفاده ازکلیدواژههای فارسی نور هالوژن، ویبروآکوستیک و کلیدواژههای انگلیسی: haloge light و Vibroacoustic از سال 1990 تا 2017 صورت گرفت و مقالاتی که در ارزیابی، بیشترین تشابه را با عنوان مطالعه مروری داشتند، وارد مطالعه شده و نتایج گزارش شد.
یافتهها: در نهایت 15 مقاله که تأثیر تحریک با نور هالوژن و ویبروآکوستیک را بررسی کردند، وارد مطالعه شدند که بین ضربان قلب جنین، تستهای ارزیابی سلامت جنین و تحریک با ویبروآکوستیک و نور هالوژن ارتباط وجود داشت (05/0p<).
نتیجهگیری: بهطور کلی با توجه به نتایج اغلب مطالعات بررسی شده، بین تأثیر تحریک با ویبروآکوستیک و نور هالوژن و تستهای سلامت و حرکات جنین ارتباط وجود دارد و با توجه به اینکه تحریک با ویبروآکوستیک و نور هالوژن باعث کاهش موارد غیرواکنشی ضربان قلب جنین میشوند، لذا از این ابزار میتوان در مراکز ارزیابی سلامت مادر به صورت ایمن و مؤثر جهت بهبود NST جنین استفاده نمود.
