مقایسه پیامدهای خروج سوند ادراری 6 ساعت و 12 تا 24 ساعت پس از سزارین
مقدمه: در مورد استفاده از سوند ادراری و زمان خارج کردن آن پس از عمل سزارین بین مراکز درمانی مختلف اتفاق نظر وجود ندارد. مطالعه حاضر با هدف بررسی پیامدهای خروج سوند ادرار 6 ساعت پس از عمل و خروج آن 12 تا 24 ساعت پس از سزارین انجام شد.
روشکار: این مطالعه کارآزمایی شاهددار تصادفی در سال 1393 بر روی 220 بیمار داوطلب سزارین در زاهدان انجام شد. شرکت کنندگان به صورت تصادفی به دو گروه 110 نفره خروج سوند ادراری 6 ساعت و 12 تا 24 ساعت پس از عمل تقسیم شدند. بیماران این دو گروه از نظر پیامدهای پس از عمل، عوارض احتمالی و نیز طول مدت بستری در بیمارستان و رضایتمندی مورد مقایسه قرار گرفتند. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 18) و آزمونهای کای دو و تی تست انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: دو گروه از نظر خصوصیات فردی و مربوط به عمل جراحی تفاوت معنیداری با هم نداشتند. در گروه خروج زودهنگام، زمان اولین ادرار کردن (05/0>p)، زمان به راه افتادن پس از عمل (05/0>p)، احساس فوریت در ادرار کردن (03/0=p)، و پیوری (001/0=p) به طور معنیداری کمتر و رضایت از زمان خارج کردن سوند (001/0>p) بیشتر از گروه خروح دیرهنگام بود، اما دو گروه از نظر دشواری در ادرار کردن (61/0=p)، تکرر ادرار (84/0=p)، سوزش ادرار (6/0=p) و طول مدت بستری (567/0=p ) اختلاف معنیداری با هم نداشتند.
نتیجهگیری: خروج سوند ادراری 6 ساعت پس از عمل سزارین در مقایسه با خروج سوند 12 تا 24 ساعت پس از عمل با پیامدهای بهتری همراه میباشد.

بررسی عوامل مؤثر بر انجام رفتارهای تغذیه صحیح در زنان: مطالعهای مبتنی بر مدل ارتقاء سلامت پندر: 1395
مقدمه: عادات غذایی غلط علت 20% کل سرطانها در کشورهای در حال توسعه است (1). تغذیه مناسب، عدم استعمال دخانیات و فعالیت بدنی منظم، اجزای اصلی پیشگیری از بیماریهای مزمن میباشند (2)، بنابراین توجه به رعایت تغذیه صحیح، امری ضروری میباشد، لذا مطالعه حاضر با هدف بررسی عوامل مؤثر بر انجام رفتارهای تغذیه صحیح در زنان انجام شد.
روشکار: این مطالعه توصیفی- مقطعی در سال 1395 بر روی 365 نفر از زنان 60-18 ساله شهر بجنورد انجام گرفت. گردآوری دادهها با استفاده از پرسشنامه مدل ارتقاء سلامت انجام گرفت. دادههای گردآوری شده شامل خصوصیات فردی، خودرکارآمدی درک شده، احساسات، منافع درک شده، موانع درک شده، تأثیر گذارندههای بین فردی، تأثیر گذارندههای وضعیتی، تعهد به طرح عمل و پیامدهای رفتاری بود. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 24) و آزمونهای تی مستقل، کای دو، ضریب همبستگی، آنالیز واریانس یک طرفه، همبستگی اسپیرمن و رگرسیون خطی انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: رفتارهای تغذیه صحیح در زنان با عواملی نظیر تحصیلات (04/0=p)، وضعیت تأهل، (03/0=p) و میزان درآمد (0001/0>p) ارتباط معنیداری داشت، در حالی که با فاکتور وضعیت اشتغال ارتباط معنیداری نداشت (007/0=p). سازههای خودکارآمدی درک شده (01/0=p)، موانع درک شده (04/0=p) و پیامدهای رفتاری (002/0=p) با رفتارهای صحیح تغذیه در زنان ارتباط معنیداری داشتند. 282 نفر (36/77%) از افراد مورد مطالعه در زمینه برنامههایی غذایی خودکارآمد بودند.
نتیجهگیری: مطالعه حاضر کارایی مدل ارتقاء سلامت را در پیشگویی اتخاذ رفتارهای صحیح تغذیه در زنان نشان داد، از این رو این مدل میتواند به عنوان چارچوبی جهت طراحی مداخلات آموزشی استفاده شود.

آموزش به منظور افزایش رضایت جنسی زنان یائسه: آموزش فردی یا گروهی؟
مقدمه: سلامت جامعه در گرو تأمین نیازهای بهداشتی، فرهنگی و اقتصادی زنان است. یائسگی بخشی از زندگی زنان است که در بهداشت باروری اهمیت ویژهای دارد و برنامه آموزشی مناسب به منظور بهبود رضایت جنسی ایشان ضروری است، لذا مطالعه حاضر با هدف مقایسه تأثیر آموزش فردی و گروهی بر رضایت جنسی زنان یائسه انجام شد.
روش کار: این مطالعه کارآزمایی بالینی تصادفی شده در سال 94-1393 بر روی 60 زن یائسه در شهرستان اندیمشک انجام شد. افراد در دو گروه 30 نفره آموزش فردی و گروهی قرار گرفتند. برنامه آموزشی فردی بهصورت 4 جلسه آموزش 60 دقیقهای (هفتهای یک بار) بهصورت فردی و برنامه آموزش گروهی در قالب 4 جلسه آموزش یک ساعته (هفتهای یک بار) در گروههای 10 نفره به مدت یک ماه اجرا شد. هر دو گروه، 1 و 4 هفته بعد از اتمام آموزش، پرسشنامه رضایت جنسی را تکمیل کردند. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 21) و آزمونهای تی مستقل، من ویتنی، آزمون طرحهای تکراری، فریدمن و آزمون کای دو انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: میانگین نمره رضایت جنسی، 4 هفته پس از آموزش در دو گروه زنان با آموزش گروهی و فردی تفاوت آماری معنیداری پیدا کرده بود (05/0p<). میانگین نمره رضایت جنسی در زنان دریافتکننده آموزش گروهی از گروه آموزش فردی بیشتر بود.
نتیجهگیری: با توجه به اینکه افزایش رضایت جنسی در آموزش گروهی نسبت به آموزش فردی بارزتر بود، توصیه میشود جهت ارتقاء کیفیت زندگی جنسی زنان در دوره یائسگی، از روش آموزشهای گروهی استفاده شود.

تأثیر تمرینات مقاومتی همراه با مصرف کورکومین بر نیمرخ لیپیدی زنان چاق
مقدمه: با توجه به ارتباط بین الگوی زندگی با سلامتی و بروز بیماریهای غیر واگیردار، تغییر الگوی زندگی میتواند اثرات مثبتی بر سلامتی داشته باشد. با اینوجود اثر الگوی زندگی و بروز بیماریها با عامل چاقی و اختلالات ناشی از آن به خوبی معلوم نیست. بر این اساس مطالعه حاضر با هدف بررسی تأثیر ترکیب تمرین مقاومتی و مصرف کورکومین بر نیمرخ لیپیدی زنان چاق و دارای اضافه وزن غیرفعال انجام شد.
روشکار: این مطالعه کارآزمایی بالینی تصادفی شده در نیمسال دوم سال تحصیلی 93-1392 بر روی 40 نفر (از دانشجویان دانشگاه آزاد اسلامی واحد مشهد) از زنان غیر فعال در دامنه سنی 35-20 سال سالم و دارای کلسترول نزدیک به دامنه خطر با شاخص توده بدنی 32-28 کیلوگرم بر متر مربع انجام شد. افراد به صورت تصادفی در چهار گروه 10 نفره کنترل، تمرین - دارونما، کورکومین و تمرین - کورکومین قرار گرفتند. گروههای مصرفکننده کورکومین، به مدت 8 هفته روزانه یک کپسول 80 میلیگرمی نانومیسیل کورکومین را مصرف کردند. گروههای تمرین نیز هفتهای 3 جلسه به مدت 8 هفته تمرین مقاومتی را اجرا نمودند. اطلاعات مربوط به نیمرخ لیپیدی با استفاده از آزمایش خون گردآوری شدند. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 18) و آزمونهای آنالیز واریانس، کروسکال والیس، آزمون شفه و آنالیز کوواریانس استفاده شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: پس از 8 هفته تمرین غلظت تریگلیسیرید در گروه تمرین همراه با مصرف کورکومین نسبت به گروه کورکومین به طور معناداری کاهش یافت (027/0=p). تفاوت معناداری در VLDL-c (389/0=p)، غلظت HDL-c (205/0=p)، LDL-c (503/0=p) و کلسترول (335/0=p) پس از ترکیب تمرین مقاومتی و مصرف کورکومین مشاهده نشد.
نتیجهگیری: تمرین مقاومتی و کورکومین اثر یکدیگر را بر کاهش نیمرخ چربی تقویت نمینمایند. همچنین پاسخ نیمرخ لیپیدی به تمرین و کورکومین مشابه نبوده و از آنجایی که هم تمرین و هم مصرف کورکومین و ترکیب آنها بر برخی شاخصهای نیمرخ لیپیدی اثر مطلوب داشت، توصیه میشود دختران دارای اضافه وزن به عنوان اقدام پیشگیرانه از هریک از این مداخلهها استفاده کنند.

بررسی شیوع مصرف گیاهان دارویی در زنان باردار مراجعه کننده به مراکز بهداشتی و درمانی شهر بجنورد
مقدمه: گیاهان دارویی به دلایل مختلف توسط زنان باردار طی دوران بارداری استفاده میشود و در باور عموم مردم، داروهای گیاهی، سالم و بدون عوارض جانبی میباشند، در حالی که این گیاهان باعث عوارض جانبی در مادر و یا تداخلات دارویی میشوند، لذا مطالعه حاضر با هدف بررسی شیوع مصرف گیاهان دارویی در زنان باردار مراجعهکننده به مراکز بهداشتی درمانی شهر بجنورد انجام شد.
روشکار: این مطالعه توصیفی- تحلیلی در سال 1394 بر روی 350 زن باردار مراجعهکننده به مراکز بهداشتی و درمانی شهر بجنورد انجام شد. اطلاعات مربوط به نوع گیاهان دارویی، دلیل مصرف، آگاهی و عملکرد افراد در زمینه مصرف گیاهان دارویی بهوسیله پرسشنامه گردآوری شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 20) و آزمونهای تی تست و کای دو انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: 222 نفر (40/63%) از واحدهای پژوهش از داروی گیاهی استفاده کرده بودند. شایعترین علل مصرف سرماخوردگی و بیشترین گیاه مورد استفاده نعنا بود. 117 نفر (40/53%) از افراد، اطلاعات خود را از خانواده و فامیل کسب کرده بودند و 139 نفر (90/62%) از افراد گیاهان دارویی مورد نیاز خود را از عطاری تهیه میکردند، بین مصرف گیاهان دارویی با شغل و تحصیلات همسر ارتباط معنادار آماری وجود داشت (05/0p<).
نتیجهگیری: با توجه به درصد بالای استفاده از گیاهان دارویی در زنان باردار شهر بجنورد و غنی بودن این منطقه از لحاظ گیاهان دارویی، آموزش و اطلاعرسانی بیشتر در مورد داروهای گیاهی توسط مراقبین سلامت به زنان باردار لازم میباشد.

مقدمه: علائم یائسگی میتواند اثرات زیانباری بر زندگی زنان گذاشته و کیفیت زندگی آنان را تحت تأثیر قرار دهد. بسیاری از زنان یائسه جهت سازگاری با علائم یائسگی، به درمانهای طب مکمل چون گیاه درمانی و استفاده از فیتواستروژنها روی آوردهاند. مطالعه حاضر با هدف ارزیابی تأثیر رازیانه به عنوان یک فیتواستروژن در درمان علائم جسمانی یائسگی انجام شد.
روشکار: این مطالعه کارآزمایی بالینی تصادفی شده در سال 1395 بر روی 90 زن یائسه 60-45 ساله در تهران انجام شد. افراد به طور تصادفی به دو گروه رازیانه و دارونما تقسیم شدند. زنان در گروه رازیانه به مدت 8 هفته روزانه 2 کپسول نرم 100 میلیگرمی رازیانه و در گروه دارونما 2 کپسول نرم 100 میلیگرمی روغن آفتابگردان را دریافت کردند. علائم جسمانی یائسگی به وسیله بخش جسمانی پرسشنامه معیار سنجش یائسگی اندازهگیری شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 14) و آزمونهای تی مستقل و تی زوجی انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: قبل از مداخله،دو گروه از نظر علائم جسمانی یائسگی مشابه بودند، اما پس از 8 هفته مداخله، بر اساس آزمون آماری تی زوجی، در گروه رازیانه، علائم جسمانی یائسگی بعد از مداخله در مقایسه با قبل از مداخله از نظر آماری کاهش معنیداری یافت (001/0p<)، اما در گروه دارونما علائم بعد از مداخله با قبل از مداخله اختلاف آماری معنیداری نداشت (05/0<p).
نتیجهگیری: رازیانه بدون هیچ عارضه جدی در کاهش علائم جسمانی یائسگی مؤثر است. تحقیقات بیشتر برای قطعی شدن نتایج حاصل از این مطالعه مورد نیاز است.

بررسی ارتباط بین پلیمورفیسم rs2488457 ژن PTPN22 و سقط مکرر
مقدمه: سقط مکرر به ختم بارداری بیش از دو بار اطلاق میشود. عوامل متعددی در ایجاد سقط مکرر دخیل هستند، وجود اختلال در عملکرد سیستم ایمنی منجر به دفع جنین میشود. آنزیم پروتئین تیروزین فسفاتاز بافت لنفاوی (Lyp) که توسط ژن PTPN22 کد میشود، نقش حیاتی در تنظیم و تعادل پاسخهای ایمنی دارد؛ بهتازگی پلیمورفیسم rs2488457که در ناحیه پروموتر این ژن قراردارد کشف شده است، محتمل است این پلیمورفیسم با تغییر در بیان ژن PTPN22 بالانس سیستم ایمنی را مختل نماید. از آنجایی که حفظ بالانس و تعادل سیستم ایمنی در بارداری نقش بسزایی دارد، مطالعه حاضر با هدف مقایسه فراوانی پلیمورفیسم rs2488457 بین دو گروه زنان مبتلا به سقط مکررو زنان سالم انجام شد.
روشکار: این مطالعه تحلیلی از نوع مورد- شاهدی در سال 1394 بر روی 200 نفر از افراد مبتلا به سقط مکرر و 200 نفر از افراد سالم مراجعه کننده به مرکز ناباروری انجام شد. جهت بررسی ژنوتایپ افراد روش PCR-RFLP و در نهایت آزمون آماری کایدو جهت بررسی فراوانی پلیمورفیسم rs2488457 انجام شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 19) و آزمون آماری کای دو انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: فراوانی پلیمورفیسم rs2488457 در زنان مبتلا به سقط مکرر بیشتر از زنان سالم بود، اما این اختلاف از نظر آماری معنادار نبود (162/0=p).
نتیجهگیری: پلیمورفیسم rs2488457 ارتباطی با سقط مکرر ندارد.

بررسی و مقایسه میزان رضایتمندی جنسی در افراد خواستار تغییر جنسیت و افراد تغییر جنسیت داده در مراجعین به پزشک قانونی استان فارس
مقدمه: مسأله رضایت از رابطه جنسی یکی از نیازهای فیزیولوژیک انسان میباشد و در تغییر جنسیت افراد مبتلا به دیسفوریای جنسی باید مدنظر قرار گیرد. مطالعه حاضر با هدف مقایسه میزان رضایتمندی جنسی در افراد خواستار تغییر جنسیت و افراد تغییر جنسیت داده در مراجعین به پزشک قانونی استان فارس انجام شد. روشکار: این مطالعه توصیفی مقایسهای در طی سالهای 93-1390 بر روی 70 نفر از بیماران مبتلا به اختلال هویت جنسی عمل شده و عمل نشده در پزشک قانونی استان فارس انجام شد. گردآوری دادهها توسط پرسشنامه رضایت جنسی لارسون انجام شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 21) و آزمون تحلیل کواریانس یکراهه انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد. یافتهها: بر اساسنتایج حاصل از آزمون آماری، میانگین نمرات رضایت جنسی گروه تغییر جنسیت داده (81/9±02/88)، از میانگین نمرات گروه خواستار تغییر جنسیت (53/10±08/76)، به طور معناداری بیشتر بود (001/0=p). بر اساس نتایج تحلیل کوواریانس یک راه، بین دو گروه تغییر جنسیت و تغییر جنسیت داده از نظر میانگین نمرات رضایت جنسی تفاوت معناداری وجود داشت (001/0=p)، همچنین تعامل بین جنسیت، سن، تحصیلات، شغل، شغل پدر و شغل مادر با متغیر مستقل در تأثیرگذاری بر متغیر وابسته رضایت جنسی معنادار نبود (05/0<p). نتیجهگیری: افرادی که خواستار تغییر جنسیت هستند وقتی تغییر جنسیت میدهند، به ثبات هویت جنسی میرسند و شرایط برای لذت بردن از رفتارهای جنسی افزایش مییابد، لذا آموزش و مشاورههای تخصصی مورد نیاز توسط روانشناسان و روانپزشکان مجرب به این دسته از افراد بسیار ضروری به نظر میرسد.

بررسی تأثیر روغن گلمغربی بر طول مدت اندوه پس از زایمان در زنان نخستزا: یک کارآزمایی بالینی دوسوکور کنترل شده
مقدمه: از آنجایی که در تحقیقات مختلف بین سطح امگا-3 و خلق افسرده ارتباط وجود دارد و روغن گلمغربی از منابع غنی امگا-3 میباشد، مطالعه حاضر با هدف بررسی تأثیر روغن گل مغربی بر طول مدت اندوه پس از زایمان زنان انجام شد. روشکار: این مطالعه کارآزمایی بالینی دوسوکور در سال 92-1391 بر روی 132 نفر از زنان باردار نخستزای مراجعهکننده به مراکز بهداشتی اهواز صورت گرفت. افراد در دو گروه 66 نفری جهت دریافت روزانه یکگرم کپسول روغن گل مغربی (گروه مداخله) و کپسول ژلاتینی خوراکی مشابه (گروه کنترل) از آغاز هفته 37 بارداری تا 2 هفته پس از زایمان قرار گرفتند. نمونهها در روزهای 4، 10 و 14 پس از زایمان با آزمون استاندارد ادینبرگ بررسی شدند. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS(نسخه 20) و آزمونهای آماری تیمستقل، کایاسکوئر و آنالیز مقادیر تکراری انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد. یافتهها: بین طول مدت اندوه پس از زایمان دو گروه اختلاف آماری معنیداری وجود داشت؛ بهطوریکه طول مدت اندوه پس از زایمان در گروه مداخله کمتر از گروه پلاسبو بود (004/0=p). میانگین نمره آزمون ادینبرگ بهطور معنیداری در گروه مداخله نسبت به گروه کنترل در روز چهارم (0001/0=p)، دهم (001/0=p) و چهاردهم (08/0=p) پس از زایمان کمتر بود. نتیجهگیری: مصرف روغن گل مغربی در کاهش طول مدت اندوه پس از زایمان مؤثر است.

بررسی مقایسهای تأثیر کپسول رزماری و مفنامیک اسید بر طول مدت دیسمنوره اولیه
مقدمه: دیسمنوره اولیه، یکی از شایعترین مشکلات زنان است که درمان آن، استفاده از داروهای ضد التهاب غیر استروئیدی است که بهدلیل داشتن عوارض زیاد از مقبولیت برخودار نمیباشند، لذا با توجه به خواص ضد درد و ضد التهابی رزماری، مطالعه حاضر با هدف مقایسه تأثیر کپسول رزماری و مفنامیکاسید بر طول مدت دیسمنوره اولیه انجام شد.
روشکار: این مطالعه کارآزمایی بالینی دوسوکور در سال 1395 بر روی 90 نفر از دانشجویان دارای دیسمنوره اولیه در دانشکده پزشکی دانشگاه آزاد اسلامی مشهد انجام شد. جهت ارزیابی شدت درد از مقیاس دیداری درد و مدت درد بر حسب دقیقه استفاده شد. واحدهای پژوهش بهطور تصادفی در دو گروه 45 نفره قرار گرفتند. افراد یک گروه کپسول رزماری 250 میلیگرمی و گروه دیگر کپسول مفنامیکاسید 250 میلیگرمی را طی دو دوره در سه روز اول قاعدگی هر 8 ساعت دریافت کردند. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 24) و آزمونهای منویتنی و ویلکاکسون انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: میانگین طول مدت درد در گروه رزماری در سیکل کنترل از 52/18±49/131 دقیقه به ترتیب به 43/18±83/95 و 32/10±76/83 دقیقه در سیکل اول و دوم مداخله تقلیل یافت و در گروه مفنامیکاسید در سیکل کنترل از 59/28±12/182 دقیقه به ترتیب به 07/30±16/140 و 44/26±04/130 دقیقه در سیکل اول و دوم مداخله تقلیل یافت که بین دو گروه در اولین و دومین سیکل مداخله از نظر آماری تفاوت آماری معنیداری وجود نداشت (05/0<p).
نتیجهگیری: مصرف کپسول رزماری در کاهش طول مدت درد دیسمنوره اولیه مؤثر است و میتوان از این گیاه به عنوان یک روش درمانی استفاده کرد.

مقایسه تأثیر دو روش آموزش سنتی (چهره به چهره) و آموزش مبتنی بر مدل فرآیند موازی توسعهیافته بر آگاهی از عوارض چاقی قبل بارداری در زنان با شاخص توده بدنی بالا
مقدمه: چاقی قبل از بارداری مادر، یک عامل خطر مستقل جهت پیامدهای سوء بارداری محسوب میشود. افزایش آگاهی و ایجاد حساسیت نسبت به این پیامدها، اولین گام جهت ارتقای رفتارهای پیشگیرانه در این افراد میباشد. مدل فرآیند موازیتوسعه یافته در پیشگیری از رفتارهای پرخطر بهداشتی از طریق افزایش آگاهی، حساسیت و خودکارآمدی افراد به عنوان مدلی موفق شناخته شده است، لذا مطالعه حاضر با هدف مقایسه تأثیر دو روش آموزش سنتی (چهرهبهچهره) و آموزش مبتنی بر مدل فرآیند موازی توسعه یافته بر آگاهی از عوارض چاقی قبل بارداری در زنان با شاخص توده بدنی بالا انجام شد. روشکار: این مطالعه کارآزمایی بالینی با نمونهگیری چند مرحلهای تصادفی در سال 1394 بر روی 60 نفر از زنان مراجعهکننده به دو مرکز بهداشت شهر مشهد جهت مشاوره قبل بارداری انجام شد. برای گروه آزمون دو جلسه آموزشی مبتنی بر مدل توسط محقق انجام شد و گروه کنترل نیز توسط مراقبین سلامت آموزش مرسوم (چهرهبهچهره) را دریافت کردند. ابزار گردآوری دادهها پرسشنامه سازههای مدل و آگاهی زنان از خطرات وزن بالا قبل از بارداری بود که قبل مداخله، 2 و 4 هفته بعد از مداخله تکمیل شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 16) و آزمونهای آماری تی مستقل، کایدو، منویتنی و آنالیز واریانس با دادههای تکراری انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد. یافتهها: میانگین نمره آگاهی در قبل از مداخله بین دو گروه اختلاف معنیداری نداشت (05/0<p)؛ اما بعد از مداخله، افزایش معنیداری در نمره آگاهی گروه آزمون نسبت به گروه کنترل بهترتیب در 2 هفته (001/0>p) و 4 هفته بعد از مداخله (001/0>p) مشاهده شد. نتیجهگیری: آموزش مبتنی بر مدل فرآیند موازی توسعه یافته میتواند در ارتقای آگاهی زنان با شاخص توده بدنی بالا موفقتر از روش مرسوم (چهرهبهچهره) عمل کند.

بررسی تأثیر طب فشاری بر انقباض رحم و دردهای بعد از زایمان
مقدمه: آتونی رحم، شایعترین علت خونریزی زایمانی است و انقباض مناسب رحم همراه با درد میباشد. طب فشاری منجر به تحریک عملکردهای فیزیولوژیک کانالها شده و در برخی بیماریها با اداره علائم و نیز با افزایش جریان انرژی باعث تعادل در عملکرد داخلی بدن میشود. مطالعه حاضر با هدف بررسی تأثیر طب فشاری بر تون رحم و دردهای بعد از زایمان انجام شد.
روش کار: این مطالعه کارآزمایی بالینی تصادفی دوسوکور در سال 1394 بر روی 114 مادر با زایمان طبیعی در بیمارستان ام البنین (س) انجام شد. مادران به صورت تخصیص تصادفی به 3 گروه طب فشاری اصلی و شم (نقاط نابجا) و کنترل تقسیم شدند. در گروه طب فشاری اصلی، بعد از تولد نوزاد و قبل از خروج جفت، فشاری به مدت 2 دقیقه بر روی نقاط سی وی 6 و 4 و در گروه طب فشاری شم، فشار کمتر روی دو نقطه نابجا اعمال گردید. گروه کنترل مداخلهای نداشتند. تون رحم بلافاصله بعد از خروج جفت و همزمان با پسدرد با مقیاس سنجش دیداری، 1 و 2 ساعت بعد از تولد توسط کمک پژوهشگر اندازهگیری شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری SPSS (نسخه 16) و آزمونهای کروسکال والیس، آنالیز واریانس و کای اسکوئر انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: تون رحم کاملاً منقبض در گروه طب فشاری اصلی در سه بار بررسی بلافاصله بعد از خروج جفت، در ساعت اول و ساعت دوم به ترتیب 5/89، 8/86، 8/86؛ در گروه طب فشاری شم 8/86، 2/84، 5/89 و در گروه کنترل 6/81، 3/76 و 9/78 بود و علی رغم اینکه در گروه اصلی نسبت به گروه کنترل بیشتر بود، ولی بر اساس آزمون کروسکال والیس در سه گروه تفاوت معنیداری نداشت (05/0<p). میانگین شدت درد در گروه طب فشاری اصلی در ساعت اول و ساعت دوم به ترتیب 5/17±9/8 و 9/11± 6/4؛ در گروه طب فشاری شم 9/15±5/2 و 9/13±3/5؛ و در گروه کنترل 2/19±8/9 و 4/17±6/7 بود و علیرغم اینکه در گروه اصلی نسبت به گروه کنترل کمتر بود، ولی بر اساس آزمون کروسکال والیس در سه گروه تفاوت معنیداری نداشت (05/0<p).
نتیجهگیری: انجام طب فشاری بر نقاط اصلی بر تون و درد 1 و 2 ساعت پس از زایمان تأثیری ندارد، ولی در خصوص اثر آن در طول زمان مورد نظر برای پس درد و تأثیر بر تون، انجام مطالعات بیشتر ضروری است.

بررسی تأثیر مدل 5آ بر تغییر رفتار فعالیت فیزیکی در زنان باردار با اضافه وزن
مقدمه: اضافه وزن و چاقی، یک چالش قابل توجه برای مراقبت دوران بارداری میباشد. حدود 50% از زنان با شاخص توده بدنی بالای 25 کیلوگرم بر متر مربع باردار میشوند که میتواند با افزایش خطر بارداری همراه باشد. مطالعه حاضر با هدف تعیین تأثیر مدل 5آ بر تغییر رفتار فعالیت فیزیکی در زنان باردار دارای اضافه وزن انجام شد.
روشکار: این مطالعه کارآزمایی بالینی در سال 95-1394 بر روی 120 زن باردار با اضافه وزن مراجعه کننده به مراکز بهداشتی درمانی ارومیه انجام شد. گروه کنترل مراقبتهای معمول را دریافت و گروه مداخله آموزش چهره به چهره فعالیت فیزیکی بر اساس مدل خودمدیریتی 5آ را دریافت کرد. این مطالعه طی 3 ماه در قالب 5 مرحله سؤال، ارزیابی، راهنمایی، توافق، کمک و پیگیری به اجرا درآمد. گردآوری دادهها با استفاده از پرسشنامه فعالیت فیزیکی در بارداری انجام گرفت. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماریSPSS (نسخه 21) و آزمونهای تیمستقل، کای دو و آنالیز کواریانس انجام شد. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد.
یافتهها: متوسط سطح کل فعالیت فیزیکی در گروه کنترل در شروع و پایان مداخله 89/38±30/74 و 67/25±37/60 و در گروه مداخله 70/15±32/88 و 14/35±30/117 مت بر دقیقه بود. در پایان مداخله اختلاف دو گروه بر اساس الگوی فعالیت فیزیکی و میزان وزنگیری معنیدار بود. متوسط فعالیت فیزیکی بین دو گروه در پایان مداخله تفاوت معنیداری داشت (001/0>p). آنالیز کوواریانس، تفاوت آماری معنیداری را بین دو گروه بر اساس فعالیت فیزیکی کل و تغییرات وزن پس از تعدیل متغیرهای مخدوشگر نشان داد (001/0>p).
نتیجهگیری: مداخله خودمدیریتی بر اساس مدل 5آ میتواند نقش مؤثری در تغییر رفتار زنان باردار داشته باشد و منجر به ایجاد انگیزه، تغییر نگرش و رفتار آنان شده و به آنها کمک میکند برای بهبود وزنگیری طبق دستورالعمل و بهدست آوردن وزن مناسب در بارداری، رفتار و سبک زندگی خود را مدیریت کنند.
