بررسی ارتباط درد بعد از سزارین با اضطراب قبل و بعد از جراحی

مقدمه: شدت درد و اضطراب در همه افراد یکسان نیست، به عبارتی تجربه درد در افراد تحت تأثیر تجارب گذشته درد، اضطراب و انتظارات بیمار از تسکین درد میباشد. لذا مطالعه حاضر با هدف بررسی ارتباط بین درد بعد از سزارین با اضطراب قبل و بعد از جراحی انجام شد. روشکار: این مطالعه همبستگی در سال 1394 بر 101 زن مراجعهکننده به بیمارستان امالبنین (س) مشهد که تحت سزارین انتخابی قرار گرفتند، انجام شد. افراد با روش نمونهگیری آسان انتخاب شدند. جهت سنجش اضطراب قبل از عمل، 3-2 ساعت قبل از جراحی، بیماران پرسشنامه اضطراب اشپیلبرگر را تکمیل کردند. 5/2-5/1 ساعت پس از جراحی، اضطراب با پرسشنامه اشپیلبرگر و درد، با پرسشنامه کوتاه شده درد مکگیل و مقیاس دیداری درد ارزیابی شد. دادهها پس از گردآوری با استفاده از نرمافزار آماری SPSS (نسخه 16) و آزمونهای کایاسکوئر و همبستگی اسپیرمن و پیرسون با کنترل زمان، مورد تجزیه و تحلیل قرار گرفت. میزان p کمتر از 05/0 معنیدار در نظر گرفته شد. یافتهها: از 101 نفر مورد مطالعه، 67 نفر (4/66%) دارای اضطراب آشکار قبل از عمل متوسط و 64 نفر (4/63%) دارای اضطراب پنهان متوسط بودند و پس از عمل 53 نفر (5/52%) از بیماران درد متوسط و 78 نفر (2/77%) اضطراب آشکار متوسط داشتند. بین اضطراب آشکار (91/0=p، 01/0-=r) و پنهان (86/0=p، 018/0=r) قبل از عمل با درد پس از عمل ارتباط معناداری مشاهده نشد؛ اما بین اضطراب آشکار بعد از عمل و شدت درد ارتباط آماری معناداری وجود داشت (001/0=p، 32/0=r). نتیجهگیری: بین اضطراب پس از سزارین با درد ارتباط وجود دارد؛ بنابراین برای کاهش درد بیمار بهتر است راهکارهای کاهش اضطراب نیز مورد توجه قرار گیرد.
Introduction: The intensity of anxiety and pain is not the same in all people. In other words, pain experience in patients is affected by past experiences of pain, anxiety and expectation of pain relief. So, this study was performed with aim to investigate the relationship between postoperative pain after cesarean section with pre and postoperative anxiety. Methods: This correlation study was conducted on 101 pregnant women referred to Mashhad Omolbanin hospital who underwent elective caesarean section in 2015. Sampling was performed by accessible method. To assess preoperative anxiety, the patients completed Spielberger questionnaire2-3 hours before surgery. 1.5-2.5 hours after surgery, anxiety was evaluated by Spielberger questionnaire and pain was measured by short form of Mcgill pain questionnaire and visual analogue scale. Data were analyzed by SPSS software (version 16), Chi-square, Spearman and Pearson correlation tests by controlling the time. PResults: from 101 studied cases, 67 patients)66.4%( had moderate state preoperative anxiety and 64 (63.4%) moderate trait preoperative anxiety and 78 (77.2%) moderate state postoperative anxiety and 53 (52.5%) had moderate postoperative pain. There was no significant correlation between preoperative state anxiety (r=-0.01 ,P=0.91) and trait anxiety (r=0.018 , P=0.86) with postoperative pain. But, significant relationship was observed between postoperative state anxiety and postoperative pain (r=0.32 ,P=0.001). Conclusion: There is a significant relationship between postoperative anxiety and postoperative pain in cesarean section. So, it is better to consider the methods of reducing anxiety to decrease the patient's pain.

بررسی نقش طب مکمل در کنترل علائم سندرم پیش از قاعدگی مبتنی بر شواهد بالینی (مطالعه مروری منظم)

مقدمه: سندرم پیش از قاعدگی یکی از شایعترین علل مراجعه زنان به مراکز درمانی میباشد، اتیولوژی آن نامشخص و روشهای درمانی به طور نسبی بر روی کنترل علائم آن مؤثر میباشد. مطالعه مروری حاضر با هدف بررسی اثربخشی طب مکمل در کنترل علائم علائم این سندرم انجام شد. روشکار: در این مطالعه مروری مقالات از طریق جستجو در پایگاههای اطلاعات علمی داخلی (جهاد دانشگاهی، سیلویکا و نشریات دانشگاهها) و پایگاههای خارجی نظیرPubMed ،Scopus وGoogle scholar با کلید واژههای سندرم پیش از قاعدگی، طب مکمل، طب سنتی ایران و گیاهان دارویی بدون اعمال محدودیت زمانی جستجو شدند. از بین 80 مقاله بهدست آمده، 39 مقاله بررسی شد. یافتهها: در تحقیقاتمختلفتغذیه با مواد مغذی از جمله ویتامین D، مواد معدنی مانند کلسیم و روی، چربیها مانند امگا-3 و گیاهان دارویی زیادی از جمله رازیانه، زعفران، کرفس، انیسون، گل مغربی و روشهای روان درمانی مبتنی بر شناخت درمانی و استفاده از تکنیکهای آرامسازی، یوگا، مراقبه و ماساژ و ورزش، برای کنترل علائم سندرم به کار رفتهاند. نتیجهگیری: کارآمدی برخی ظرفیتهای طب مکمل به همراه استفاده از ظرفیتهای درمانی طب کلاسیک در سندرم پیش از قاعدگی میتواند الگوی جدیدی برای بررسی و درمان این مشکل فراگیر فراهم کند. مطالعات مروری و بالینی که بر این اصول طراحی شوند، میتوانند در توسعه رویکرد طب تلفیقی و ارتقاء سلامت زنان مؤثر باشند.
Introduction: Premenstrual syndrome (PMS) is one of the most common causes of women’s refering to medical care. Its etiology is unknown and treatment methods are relatively effective to control its symptoms. This study was performed with aim to evaluate the effectiveness of complementary medicine in controlling the symptoms of PMS. Methods: This review study, the articles were searched in Iranian databases: SID, cilivika, and universities’ journals, and international databases: PubMed, Scopus, Google Scholar using the key words of premenstrual syndrome, complementary medicine, Iranian Traditional Medicine, and medicinal plants with no time limitation. 39 articles of 80 papers obtained were evaluated. Results: In different studies, nutrition with nutrients such as vitamin D, minerals including calcium and zinc, fats like omega-3, and medicinal plants including fennel, saffron, celery, anise, evening primrose and psychotherapy methods based on cognitive therapy and using the relaxation techniques, yoga, meditation, massage and sports are used for controlling thr syndrome’s symptoms. Conclusion: The effectiveness of some potentials of complementary medicine along with therapeutic potentials of classic medicine in premenstrual syndrome can provide a new pattern for evaluation and treatment of this widespread problem. Review and clinical studies which be designed based on these principles can be effective on the development of integrated medicine approach and women's health promotion.

مقدمه: اولین و متحملترین علامت پارگی رحم، الگوی غیر اطمینان بخش ضربان قلب جنین است که میتواند باعث مرگ جنین شود. در موارد مرگ جنین داخل رحمی امکان تشخیص زودرس پارگی رحم بر اساس الگوی قلب جنین امکانپذیر نیست. در این مطالعه یک مورد نادر خروج امنتوم از واژن به دنبال زایمان طبیعی بعد سزارین یک جنین مرده گزارش شد. معرفی بیمار: زن 31 ساله G2L1 با سن حاملگی 38 هفته با شکایت درد زایمانی به اورژانس زایشگاه یکی از بیمارستانهای آموزشی زنان مشهد مراجعه کرد. در بدو مراجعه به اورژانس، قلب جنین سمع نشد. انقباضات خود به خودی مادر برای پیشرفت زایمانی مناسب بود. با توجه به کاهش انقباضات مؤثر، اکسیتوسین تجویز شد و در نهایت با توجه به توقف نزول و مرگ جنین و عدم رضایت مادر به سزارین، زایمان با وکیوم انجام شد. قبل از بررسی محل انسزیون سزارین امنتوم داخل واژن لمس شد. بیمار با تشخیص پارگی رحم منتقل اتاق عمل شد. لاپاراتومی با انسزیون فان اشتیل در محل اسکار قبلی انجام شد که پارگی رحم در محل اسکار قبلی با گسترش به سگمان تحتانی مشهود بود که ترمیم شد. نتیجهگیری: در زایمانهایی که مرگ داخل رحمی اتفاق افتاده است با توجه به عدم دسترسی به قلب جنین به عنوان علامت خطر پارگی رحم توجه به سیر لیبر اهمیت ویژهای دارد و در صورت کاهش انقباضات زایمانی و یا عدم نزول سر بخصوص در فاز فعال زایمانی باید از تجویز اکسی توسین خودداری کرده و اقدام به عمل جراحی کرد.
Introduction: The first and most likely sign of uterine rupture is non-reassuring monitoring of fetal heart rate which can lead to fetal death. In cases of intrauterine fetal death, early diagnosis of uterine rupture based on fetal heart rate monitoring is not possible. The aim of this study is to report a rare case of omentum prolapse from vagina after vaginal delivery following cesarean of a fetal death. Case Report: A 31 years old woman, G2L1 with gestational age of 38 weeks with labor pain referred to emergency maternity of one of the academic hospitals of Mashhad. Fetal heart was not detected at admission. Spontaneous contraction was appropriate for labor progress. Oxytocin was administered according to decreased contractions. Regarding to head undescending and fetal death and mother's no consent for cesarean, delivery was performed by vacuum. Omentum in vagina was touched before examining the cesarean incision. Patient was transferred to the operating room with a diagnosis of uterine rupture. Laparotomy was performed with fannenstiel incision and rupture was detected at previous cesarean scar with extension to inferior segment that suture was done. Conclusion: In cases of intrauterine fetal death, since fetal heart rate is not accessible as a risk factor for uterine rupture, it is important to notice labor progress; and in the case of reduced contractions or head undescending especially in active phase of labor, oxytocin administration should be avoided and surgery should be performed.
