Since pregnancy is a known trigger of TTP attacks, pregnant women are at risk of recurrence. Prompt diagnosis and treatment before 20 weeks gestation is important and is associated with positive outcomes. In this study, a pregnant woman with TTP in her second and third pregnancies is introduced. This study was performed with aim to introduce the patient and review the most recent management and treatment of TTP in pregnancy and prenatal counseling in patients who have a history of TTP. Case presentation: The patient is a 28-years-old woman (g3p1d1ab1). Her first pregnancy was ended with spontaneous abortion at 6 weeks and in second pregnancy, she had a cesarean section at 28 weeks gestation due to HELLP syndrome and eclampsia. After the surgery with diagnosis of TTP, plasmapheresis was performed. In current pregnancy at 36 weeks gestation, cesarean section was performed with primary diagnosis of HELLP syndrome; due to a drop in platelet count after surgery and observation of schistocytosis in peripheral blood smear, with diagnosis of TTP, daily plasmapheresis was done until the platelet count achieved above 100000 and the patient was discharged with good general condition. Conclusion: Since pregnancy is a known trigger of TTP episodes, follow-up of patients with TTP in next pregnancy and control of 13 factor and differentiating of congenital and acquired disease is essential; thus the risk of recurrence of maternal and fetal complications are greatly reduced.}, keywords = {complication,Thrombotic thrombocytopenic purpura,pregnancy,Treatment}, title_fa = {پورپورای ترومبوسیتوپنیک ترومبوتیک حاملگی و پیامدهای بارداری بعدی: گزارش یک مورد}, abstract_fa = {مقدمه: پورپورای ترومبوسیتوپنیک ترومبوتیک (TTP) یک بیماری تهدید کننده حیات، با شیوع بالای 5% در حاملگی می‌باشد که باعث ترومبوسیتوپنی می‌شود. از آنجا که حاملگی یک محرک شناخته شده TTP است، زنان باردار در معرض خطر عود هستند. تشخیص سریع و درمان بیماری قبل از هفته 20 بارداری اهمیت دارد و با پیامدهای مثبت بارداری همراه است. در این مقاله زن بارداری با ابتلاء به TTP در حاملگی دوم و سوم معرفی می‌شود. این مطالعه با هدف معرفی بیمار، مروری بر جدیدترین بررسی‌های لازم و درمان بیماران TTP در حاملگی و نحوه کنترل و مشاوره‌های قبل از بارداری در بیماران دارای سابقه TTP انجام شد. معرفی بیمار: بیمار خانم 28 ساله G3p1d1ab1 که حاملگی اول بیمار سقط 6 هفته خود بخودی بوده و در حاملگی دوم در هفته 28 بارداری با تشخیص سندرم HELLP و اکلامپسی سزارین شده است. بیمار بعد از عمل به دنبال مسجل شدن تشخیص TTP تحت پلاسمافرز قرار گرفته بود. در حاملگی فعلی در سن 36 هفته بارداری با تشخیص اولیه  HELLP سزارین شد و بعد از عمل با توجه به افت پلاکت و رؤیت شیستوسیت در لام خون محیطی و تشخیص TTP، پلاسمافرز روزانه تا زمان رسیدن پلاکت به بالای 100 هزار انجام و بیمار با حال عمومی‌خوب ترخیص شد. نتیجه‌گیری: از آنجا که حاملگی یکی  از مهم‌ترین عوامل محرک عود TTP است، پیگیری بیماران با سابقه TTP در بارداری بعدی و کنترل فاکتور  ادامز 13 و افتراق نوع مادرزادی و اکتسابی بیماری ضروری است و با شروع درمان خطر عود و عوارض مادری و جنینی بسیار کاهش می‌یابد.  }, keywords_fa = {بارداری,پورپورای ترومبوسیتوپنیک ترومبوتیک,درمان,عوارض}, url = {https://ijogi.mums.ac.ir/article_7995.html}, eprint = {https://ijogi.mums.ac.ir/article_7995_343462460fcc78054a1abf45e2f835bc.pdf} }
