In cases of intrauterine fetal death, early diagnosis of uterine rupture based on fetal heart rate monitoring is not possible. The aim of this study is to report a rare case of omentum prolapse from vagina after vaginal delivery following cesarean of a fetal death. Case Report: A 31 years old woman, G2L1 with gestational age of 38 weeks with labor pain referred to emergency maternity of one of the academic hospitals of Mashhad. Fetal heart was not detected at admission. Spontaneous contraction was appropriate for labor progress. Oxytocin was administered according to decreased contractions. Regarding to head undescending and fetal death and mother's no consent for cesarean, delivery was performed by vacuum. Omentum in vagina was touched before examining the cesarean incision. Patient was transferred to the operating room with a diagnosis of uterine rupture. Laparotomy was performed with fannenstiel incision and rupture was detected at previous cesarean scar with extension to inferior segment that suture was done. Conclusion: In cases of intrauterine fetal death, since fetal heart rate is not accessible as a risk factor for uterine rupture, it is important to notice labor progress; and in the case of reduced contractions or head undescending especially in active phase of labor, oxytocin administration should be avoided and surgery should be performed.}, keywords = {Fetal death,Omentum,Vaginal Birth after Cesarean}, title_fa = {پرولاپس امنتوم از واژن به دنبال زایمان طبیعی بعد سزارین یک جنین مرده: گزارش مورد}, abstract_fa = {مقدمه: اولین و متحمل‌ترین علامت پارگی رحم، الگوی غیر اطمینان بخش ضربان قلب جنین است که می‌تواند باعث مرگ جنین شود. در موارد مرگ جنین داخل رحمی امکان تشخیص زودرس پارگی رحم بر اساس الگوی قلب جنین امکان‌پذیر نیست. در این مطالعه یک مورد نادر خروج امنتوم از واژن به دنبال زایمان طبیعی بعد سزارین یک جنین مرده گزارش شد. معرفی بیمار: زن 31 ساله G2L1 با سن حاملگی 38 هفته با شکایت درد زایمانی به اورژانس زایشگاه یکی از بیمارستان‌های آموزشی زنان مشهد مراجعه کرد. در بدو مراجعه به اورژانس، قلب جنین سمع نشد. انقباضات خود به خودی مادر برای پیشرفت زایمانی مناسب بود. با توجه به کاهش انقباضات مؤثر، اکسی‌توسین تجویز شد و در نهایت با توجه به توقف نزول و مرگ جنین و عدم رضایت مادر به سزارین، زایمان با وکیوم انجام شد. قبل از بررسی محل انسزیون سزارین امنتوم داخل واژن لمس شد. بیمار با تشخیص پارگی رحم منتقل اتاق عمل شد. لاپاراتومی با انسزیون فان اشتیل در محل اسکار قبلی انجام شد که پارگی رحم در محل اسکار قبلی با گسترش به سگمان تحتانی مشهود بود که ترمیم شد. نتیجه‌گیری: در زایمان‌هایی که مرگ داخل رحمی اتفاق افتاده است با توجه به عدم دسترسی به قلب جنین به عنوان علامت خطر پارگی رحم توجه به سیر لیبر اهمیت ویژه‌ای دارد و در صورت کاهش انقباضات زایمانی و یا عدم نزول سر بخصوص در فاز فعال زایمانی باید از تجویز اکسی توسین خودداری کرده و اقدام به عمل جراحی کرد.}, keywords_fa = {امنتوم,جنین مرده,زایمان طبیعی بعد سزارین}, url = {https://ijogi.mums.ac.ir/article_7745.html}, eprint = {https://ijogi.mums.ac.ir/article_7745_1568862db2ec8d0e7b4b1e0214fd9398.pdf} }
