But, it should be noted that infection due to caesarean section is one of the major cause of maternal morbidity and antibiotics are effective in prophylaxis of infection. This study was performed with aim to determine the necessary duration of antibiotic therapy to reduce infections in cesarean section. Methods: This randomized clinical trial, case-control study was performed on 204 women who were candidate for non-emergency cesarean section in Shohadaye Khalije Fars, Bushehr from 2014 to 2015. They were randomly divided to intervention (received single dose of prophylactic antibiotic after clamping umbilical cord) and control (received single dose of prophylactic antibiotic plus seven days of antibiotic after surgery) groups. They were assessed during 48 h after hospitalization in terms of fever, chills, cellulitis, discharge from incision site, uterine tenderness and they were also followed one week after discharge by referring to the clinic or hospital. Their infants were also evaluated during mother's hospitalization and also when referring by mother in terms of neonatal outcomes such as diarrhea, rash, and oral candidiasis. Data analysis was performed by SPSS software (version 18) and independent t-test, Chi-square and Fisher's exact test. PResults: There was no significant difference between the intervention and control groups in term of maternal complications including urinary tract infection by urine analysis (P=0.118), urine culture (P=0.180), metritis (P=0.622) and surgical site infection (P=0.076). Neonatal complications including diarrhea (P=0.621), rash (P=1).and oral candidiasis (P=1) has no significant difference between intervention and control groups. Conclusion: Single dose antibiotic can be used instead of seven day regimen of antibiotics to reduce the infection in non-emergency cesarean sections.}, keywords = {Antibiotic,Cesarean section,Infection,Morbidity,prophylaxis}, title_fa = {بررسی مقایسه پروفیلاکسی تک دوز سفازولین با رژیم توأم تک دوز سفازولین و تجویز هفت روزه سفالکسین در بروز عفونت های پس از عمل سزارین غیر اورژانس و عوارض زودرس آنتی بیوتیک در نوزادی}, abstract_fa = {مقدمه: سزارین از آن دسته اعمال جراحی است که نقش مهمی در کاهش بروز برخی عوارض در مادر و جنین در زایمان‌های خاص دارد. اما باید توجه داشت که عوارض عفونی پس از آن از علل اساسی ناخوشی‌های مادر هستند و مصرف آنتی‌بیوتیک می‌تواند در کاهش این عوارض مؤثر باشد. لذا مطالعه حاضر برای تعیین مدت تجویز آنتی بیوتیک برای کاهش عوارض عفونی در عمل های جراحی سزارین انجام شد. روش‌کار: این مطالعه کارآزمایی بالینی مورد شاهدی تصادفی‌شده در سال 94-1393 بر روی 204 نفر از بیماران داوطلب عمل جراحی سزارین غیر اورژانس در بیمارستان شهدای خلیج فارس بوشهر انجام شد. افراد به صورت تصادفی به دو گروه آزمون (دریافت‌کنندگان تک دوز آنتی‌بیوتیک پروفیلاکتیک پس از کلامپ بند ناف) و کنترل (دریافت‌کنندگان تک دوزآنتی‌بیوتیک همراه با آنتی‌بیوتیک 7 روزه پس از عمل) تقسیم شدند. بیماران در طی 48 ساعت بستری پس از سزارین از لحاظ علائم عفونت شامل تب، لرز، سلولیت محل برش جراحی، ترشحات محل برش جراحی و حساسیت رحمی مورد بررسی قرار گرفتند و همچنین یک هفته پس از ترخیص نیز از طریق مراجعه به درمانگاه یا بیمارستان پیگیری شدند. نوزادان آن‌ها نیز در طی مدت بستری مادر و هم چنین پس از مراجعه مجدد با مادر از نظر عوارض نوزادی از جمله اسهال، راش و کاندیدا دهانی نیز بررسی شدند. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری SPSS (نسخه 18) و آزمون‌های تی مستقل، کای دو و تست دقیق فیشر انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد. یافته‌ها: بین گروه آزمون و کنترل تفاوت معناداری از نظر بروز عوارض مادری شامل تجزیه کامل ادرار (118/0=p)، کشت ادرار (180/0=p)، متریت (622/0=p) و عفونت محل زخم جراحی (076/0=p) وجود نداشت. بروز عوارض نوزادی شامل اسهال (621/0=p)، راش (1=p) و کاندیدای دهانی (1=p) نیز بین دو گروه تفاوت معناداری نداشت. نتیجه‌گیری: در عمل‌های سزارین غیر اورژانس می‌توان از رژیم تک دوز آنتی‌بیوتیک به جای رژیم 7 روزه جهت کاهش میزان عفونت‌ها استفاده کرد.}, keywords_fa = {آنتی­بیوتیک,پروفیلاکسی,عفونت,عمل سزارین,موربیدیتی}, url = {https://ijogi.mums.ac.ir/article_7395.html}, eprint = {https://ijogi.mums.ac.ir/article_7395_4f51f23919c97d034c8b8aa247f67f6f.pdf} }
