By using the late cord clamping, neonate receives more placental transfusion that may affect the Apgar score. This study was designed to compare the effect of early and late cord clamping on the Apgar score of the neonate. Methods and Materials: This double blind, randomized, clinical trial carried out, in the year 2002 at Deziani Hospital of Gorgan, Iran. This study was approved by the local committee of medical ethics. Healthy full-term vaginally born neonates (n=64), were randomly allocated to early (30s) and late (3min) umbilical cord clamping group. During this period of time, the attendant held the neonate supine at the level of introitus. At the end of 1min and 5mins after birth The Apgar score of the neonate, residual placental blood volume and estimated neonatal blood volume were determined. Data obtained by interview and observation, analyzed by SPSS software, Mann Whitney, t student and Chi-squared tests. Results: There was a significant difference between early and late umbilical cord clamping groups (p<0.001), in the 1min and 5mins Apgar scores of the neonate. Placental residual blood volume in the late cord clamping group was 39.5% lower than the early cord clamping group (p<0.001). Estimated neonatal blood volume in late cord clamping group was 7.1% higher than early cord clamping group (p<0.001). Conclusion: Late cord clamping leads to a significant increase in the Apgar score of the neonate and the estimated neonatal blood volume. However, the placental residual blood volume significantly decreased. Further trials should delay cord clamping until cessation of cord pulse or alter the level that the newborn is kept.}, keywords = {Clamping,Apgar,neonate}, title_fa = {مقایسه تاثیربستن زودرس و دیررس بندناف بر آپگار نوزاد}, abstract_fa = {مقدمه: پس از سال ها بحث و گفتگو٬ توافق اندکی در مورد زمان مناسب برای بستن بندناف وجود دارد. به دنبال بستن دیررس بندناف٬ نوزاد تزریق خون جفتی بیشتری دریافت می کند که ممکن است بر آپگار او مؤثر باشد. این مطالعه با هدف مقایسه تاثیر بستن زودرس و دیررس بندناف بر آپگار نوزاد انجام شد.   روش کار: پژوهش حاضر به شیوه کارآزمایی بالینی شاهددار تصادفی و دو سو کور بر روی 64 نوزاد رسیده سالم، در فاصله مهر ماه تا بهمن ماه 1381 در بیمارستان دزیانی گرگان، صورت گرفت. مادران واجد شرایط قبل از زایمان طبیعی به طور تصادفی به دو گروه بستن زودرس و دیررس بندناف تقسیم شدند. بندناف در گروه بستن زودرس30 ثانیه و در گروه بستن دیررس 3 دقیقه پس از تولد در حالی که نوزاد هم سطح با دهانه واژن قرار داشت بسته شد. نمره آپگار نوزاد در پایان دقیقه اول و پنجم تولد٬ حجم خون باقی مانده جفتی و حجم تقریبی خون نوزاد تعیین شد. جهت گردآوری داده ها از فرم های انتخاب نمونه٬ مصاحبه و مشاهده استفاده گردید. داده ها با استفاده از نرم افزار SPSS و آزمون های من ویتنی٬ کای دو و تی دانشجویی تجزیه و تحلیل شد.   نتایج: نمره آپگار دقیقه اول و دقیقه پنجم بین دو گروه بستن زودرس و دیررس بندناف اختلاف معنی دار داشت(p<0.001). حجم خون باقی مانده جفتی در گروه بستن دیررس بندناف 5/39 % کمتر از گروه بستن زودرس بود(p<0.001). حجم تقریبی خون نوزاد در گروه بستن دیررس بندناف 1/7% بالاتر از گروه بستن زودرس بود(p<0.001).   نتیجه گیری: بستن دیررس بندناف به طور معنی دار سبب افزایش نمره آپگار٬ کاهش حجم خون باقی مانده جفتی و افزایش حجم تقریبی خون نوزاد می شود. مطالعات بیشتر با تغییر در زمان بستن بندناف یا سطح نگه داری نوزاد پیشنهاد می گردد}, keywords_fa = {بستن زودرس بندناف,بستن دیررس بندناف,آپگار,نوزاد}, url = {https://ijogi.mums.ac.ir/article_5931.html}, eprint = {https://ijogi.mums.ac.ir/article_5931_d2b9aae771c6e5fb5cb9e4c90ff2f91f.pdf} }
