Amongst different methods of diagnosis of bacterial vaginosis Amsel`s clinical criteria and Nugent scoring is of the most preferentiality. The aim of this study is to evaluate the diagnostic values of whole Amsel`s clinical criteria, each of Amsel`s criterion solitarily and combination of two criteria, respectively, for diagnosis of bacterial vaginosis. Methods: This study is carried out on 105 women referring to gynecologic clinic of 17-Shahrivar Hospital (Mashhad, Iran, 2006-2007) who underwent speculum examination and were qualified to enroll in study. The Nugent scoring system was used as the gold standard method for diagnosis of bacterial vaginosis. Sensitivity, specificity, positive predictive value and negative predictive value of whole Amsel`s clinical criteria, each of Amsel`s criterion solitarily and combination of two criteria for the diagnosis of bacterial vaginosis were determined according to the aimed standard. Results: Among Amsel’s clinical criteria, the most sensitive individual criterion was clue cells (96.9%) and the criterion with the highest specificity was Whiff test (80.8%). PH plus clue cells and clue cells plus vaginal discharge were the most sensitive coupled combinations (84.4%), whereas the combination of clue cells and Whiff test was of the highest specificity (89%). Conclusion: The clinical criteria for the diagnosis of bacterial vaginosis can be simplified into two clinical criteria with no considerable decline in their sensitivity and specificity.}, keywords = {Bacterial vaginosis,Amsel`s criteria,Nugent scoring system,Gram stain}, title_fa = {بررسی ارزش تشخیصی معیارهای بالینی آمسل (Amsel) در تشخیص واژینوز باکتریال}, abstract_fa = {مقدمه: شایع ترین مشکل ژنیکولوژی که زنان به دلیل آن در جستجوی درمان بر می آیند، واژینیت است. از بین روش های تشخیصی واژینوز باکتریال معیارهای بالینی آمسل (Amsel) و سیستم درجه بندی ناجنت (Nugent) روش هایی هستند که بیش از سایر روش ها مورد استفاده قرارمی گیرند. هدف از مطالعه حاضر، تعیین ارزش تشخیصی کل معیار بالینی آمسل، هریک از معیارهای آن به تنهایی و ترکیب دو معیار با هم در تشخیص واژینوز باکتریال می باشد. روش کار: پژوهش حاضر مطالعه ای است از نوع روایی آزمون های تشخیصی که بر روی 105 نفر از زنان مراجعه کننده به درمانگاه زنان بیمارستان 17 شهریور مشهد در سا ل 1387- 1386 که به هر علت، تحت معاینه لگنی قرار گرفته و شرایط ورود به پژوهش را داشتند، انجام شد. در این مطالعه درجه بندی ناجنت به عنوان استاندارد طلایی تشخیص در نظر گرفته شده و حساسیت، ویژگی، ارزش اخباری مثبت و منفی کل معیار بالینی آمسل، هر یک از معیارهای آن به تنهایی و ترکیب دو معیار با هم در تشخیص واژینوز باکتریال نسبت به آن استاندارد، محاسبه شد. یافته ها: وجود سلول های کلیدی (Clue Cells) در نمونه مرطوب واژن به تنهایی بیش ترین حساسیت (9/96%) و تست ویف (Whiff test) به تنهایی بیش ترین ویژگی (8/80% ) را در بین معیارهای بالینی آمسل به خود اختصاص داد. در ترکیب دو معیار باهم بیش ترین حساسیت مربوط به ترکیب دو معیار pH و سلول کلیدی، سلول کلیدی و ترشح یکنواخت بود (4/84%) و بیش ترین ویژگی را ترکیب دو معیار سلول کلیدی وتست ویف به خود اختصاص می داد (89 %). نتیجه‌گیری: استفاده از دو معیار بالینی آمسل می تواند به اندازه کل معیار در تشخیص واژینوز باکتریال مفید باشد، بدون این که حساسیت و ویژگی آن کاهش یابد.}, keywords_fa = {واژینوز باکتریال,معیار آمسل,درجه بندی ناجنت,رنگ آمیزی گرم}, url = {https://ijogi.mums.ac.ir/article_5871.html}, eprint = {https://ijogi.mums.ac.ir/article_5871_b6b417f7b9e64aa384db2265ac1a5081.pdf} }
