Due to the effect of radiation on cervical mucosa, evaluation of response to radiotherapy with pap smear is controversial.  This study was performed with the aim to evaluate the value of post-radiation pap smear as a predictive factor of tumor response. Methods: This cross-sectional study was performed during three years (2005-2008) on 30 patients with stage IIB cervical cancer who referred to oncology department of Mashhad Ghaem and Omid hospitals. Patients who continued their follow up with pap smear and gynecologic exam at the end of radiotherapy, 1, 3 and 24 months later were entered to the study. Response to radiotherapy based on pap smear and gynecologic exam at different times after the end of radiation were compared with each other and also were compared with the results of similar evaluations at the second year. Those with abnormal pap smear and diagnosis of recurrence at the end of third month were excluded from the study. Results: According to complete concordance between pap smear and clinical exam 24 months after radiotherapy, we assumed this time as the reference for response. The concordance rate of pap smear results at the end of radiation treatment,1 month and three months after treatment, with pap smear results 24 months after radiation were 70% (p=0.088), 50% (p=0.043) and 97% (0.033), respectively. The pap smear had 100% sensitivity, 95.8% specificity and 97% diagnostic accuracy at the end of third months after radiation. Conclusion: Pap smear at the end of radiotherapy did not indicate treatment failure or poor patient outcome. Otherwise, Pap smear 3 months after radiation can be used as a predictive factor for response to treatment.}, keywords = {Cervical Cancer,Pap smear,Radiotherapy}, title_fa = {پاپ اسمیر ماه سوم پس از رادیوتراپی؛ فاکتور پیشگویی کننده پاسخ به درمان در سرطان سرویکس}, abstract_fa = {مقدمه: سرطان سرویکس، شایع ترین سرطان ژنیکولوژی است. به دلیل اثرات رادیاسیون بر مخاط سرویکس، ارزیابی پاسخ به رادیوتراپی بر اساس پاپ اسمیر، مسئله ای مورد بحث است. مطالعه حاضر با هدف بررسی ارزش پاپ اسمیر پس از رادیوتراپی به عنوان یک عامل پیشگویی کننده پاسخ به تومور انجام شد. روش کار: این مطالعه مقطعی سه ساله در سال 2008-2005 بر روی 30 بیمار مبتلا به مرحله IIB سرطان سرویکس، مراجعه کننده به دو بخش انکولوژی بیمارستان های قائم و امید مشهد انجام شد. بیمارانی که پیگیری با پاپ اسمیر و معاینه ژنیکولوژی را در انتهای درمان، یک ماه بعد، 3 ماه بعد و 2 سال بعد انجام داده بودند وارد مطالعه شدند. پاسخ به رادیوتراپی بر اساس پاپ اسمیر و معاینه ژنیکولوژی در زمان های مختلف پس از اتمام رادیوتراپی با یکدیگر و با نتایج ارزیابی های مشابه در پایان 2 سال مقایسه شد. بیماران با پاپ اسمیر غیرطبیعی و تشخیص عود در پایان سه ماه از مطالعه خارج شدند. داده ها پس از گردآوری با استفاده از نرم افزار آماری SPSS (نسخه 17) مورد تجزیه و تحلیل آماری قرار گرفت. میزان p کمتر یا مساوی 05/0 معنی دار در نظر گرفته شد. یافته ها: به دلیل تطابق کامل میان پاپ اسمیر و معاینه بالینی 24 ماه پس از رادیوتراپی، پاپ اسمیر این زمان به عنوان معیار پاسخ در نظر گرفته شد. میزان تطابق پاپ اسمیر اتمام درمان، یک ماه بعد و 3 ماه بعد با پاپ اسمیر 24 ماه پس از رادیوتراپی به ترتیب 70% (088/0=p)، 50% (043/0=p) و 97% (033/0=p) بود. حساسیت، ویژگی و صحت تشخیصی پاپ اسمیر در پایان ماه سوم به ترتیب 100%، 8/95% و 97% بود. نتیجه گیری: پاپ اسمیر اتمام رادیوتراپی، مؤید شکست درمان یا پیش آگهی بد بیمار نیست. در مقابل پاپ اسمیر سه ماه پس از رادیوتراپی را می توان به عنوان یک عامل پیشگویی کننده پاسخ به درمان به کار برد.  }, keywords_fa = {پاپ اسمیر,رادیوتراپی,سرطان سرویکس}, url = {https://ijogi.mums.ac.ir/article_3775.html}, eprint = {https://ijogi.mums.ac.ir/article_3775_3fc2efc0585970f2429356f2e346a0e0.pdf} }
