There is little information about the effects of these drugs in this group of women. This study was performed with the aim to compare the therapeutic effects of glibenclamide and insulin in treatment of gestational diabetes. Methods: This clinical randomized interventional study was performed on 90 women with gestational diabetes requiring treatment in 2010-2011. Women were randomly assigned to treatment with glibenclamide or insulin. Subcutaneous insulin 20 units twice a day was prescribed and, if needed, increased weekly. In the other group, 2.5 mg glibenclamide orally, once a day was started and if needed increased weekly up to a maximum dose of 20 mg. Treatment goal was fasting blood sugar 60-90 and after the food less than 120 mg/dl. Data were analyzed by SPSS software version 16 and Chi-square, independent t, Fisher exact and Mann-Whitney tests. PResults: Only one of the women in the glibenclamide group required to insulin for controlling blood sugar. There was no significantly difference between two groups in the percentage of infants with macrosomia (0.649), Oligohydramnios (0.304), policitemia (0.304), hypoglycemia (0.325), Polyhydramnios (0.778) and other neonatal complications. Conclusion: Glibenclamide has equivalent effectiveness with insulin in the treatment of gestational diabetes.}, keywords = {Gestational diabetes,glibenclamide,Insulin}, title_fa = {مقایسه اثرات درمانی انسولین و گلی بن کلامید در دیابت بارداری}, abstract_fa = {مقدمه: زنان مبتلا به دیابت بارداری، به دلیل نگرانی در مورد بروز ناهنجاری در جنین و نیز هیپوگلیسمی نوزادان به ندرت با سولفونیل اوره ها درمان می شوند. اطلاعات اندکی در مورد تأثیر این داروها در این گروه از زنان وجود دارد. مطالعه حاضر با هدف مقایسه اثرات درمانی گلی‌ بن‌کلامید و انسولین در درمان دیابت بارداری انجام شد. روش کار: این مطالعه کارآزمایی بالینی مداخله گر در سال 90-1389 بر روی 90 زن مبتلا به دیابت بارداری که نیاز به درمان داشتند انجام شد. زنان به طور تصادفی تحت درمان با گلی بن کلامید و یا انسولین قرار گرفتند. انسولین زیر جلدی 20 واحد 2 بار در روز تجویز شد و در صورت نیاز، به صورت هفتگی افزایش یافت. در گروه دیگر 5/2 میلی گرم گلی بن کلامید خوراکی، یک بار در روز شروع شد و در صورت نیاز هفتگی تا حداکثر دوز 20 میلی گرم افزایش یافت. هدف درمان، قند خون ناشتای 90-60 و پس از غذای کمتر از 120 میلی گرم در دسی لیتر بود. تجزیه و تحلیل داده ها با استفاده از نرم‌افزار SPSS (نسخه 16) و آزمون های کای دو، تست دقیق فیشر، تی مستقل و من ‌ویتنی انجام شد. میزان p کمتر از 05/0 معنی ‌دار در نظر گرفته شد. یافته ها: تنها یکی از زنان در گروه گلی‌ بن‌ کلامید جهت کنترل قند خون نیازمند دریافت انسولین شد. دو گروه گلی ‌بن‌ کلامید و انسولین از نظر درصد نوزادان مبتلا به ماکروزومی (649/0)، اولیگوهیدروآمینوس (304/0)، پلی ‌سیتمی (304/0)، هیپوگلسیمی (325/0)، پلی ‌هیدروآمینوس (778/0) و سایر عوارض نوزادی تفاوت معنی ‌داری نداشتند. نتیجه گیری: گلی ‌بن ‌کلامید در درمان دیابت بارداری دارای اثربخشی معادل درمان با انسولین می باشد.}, keywords_fa = {انسولین,دیابت بارداری,گلی ‌بن ‌کلامید}, url = {https://ijogi.mums.ac.ir/article_3618.html}, eprint = {https://ijogi.mums.ac.ir/article_3618_125d093c125e87eb3da061a9ab2d3b50.pdf} }
