Studies were demonstrated, treatment with levothyroxine in pregnant women with thyroid peroxides antibodies may prevent maternal and fetal complications. This study illustrated the prevalence of thyroid peroxides antibodies and induced pregnancy complications and then evaluates the effects of levothyroxine in reducing pregnancy complications among pregnant women having such antibodies. Methods and materials: we designed single blind RCT study in 300 first trimester pregnant women attending Prenatal Clinic in Sanandaj from September 2009 to January 2011. of 266 pregnant women without clinical hypothyroidism, 32 women were thyroid peroxides antibody positive. This 32 thyroid peroxides antibody positive women were divided in two groups via simple randomized allocation. Intervention group received a daily dose of 50 microgram levothyroxine in the pregnancy period. Control group did not receive any treatment. Participants were observed until the end of pregnancy period and outcome was assessed. Then data was analyzed by SPSS 11.5 software and Chi square, Fisher and Kruskal-Wallis test. P-values less than 0.05 were considered statistically significant. Results: of 266 pregnant women, 32 women (12%) were thyroid peroxides antibody positive. There were no significant statistical differences between delivery type and delivery time. (p=0.37) Though premature delivery and premature rupture of membrane in treated group was lower than non-treated group and the average of newborns weight and newborns Apgar score was higher than non-treated group, this difference was not statistically significant. (p=0.22, p=0.34 and p=0.16) Conclusion: Add tests for thyroid peroxidase antibodies to pregnancy care and treatment with levothyroxine in pregnant women with thyroid peroxides antibodies can reduce premature delivery and premature rupture of membrane and increase the average of newborns weight and newborns Apgar score.}, keywords = {thyroid peroxides antibody,Levothyroxine,Abortion,PROM}, title_fa = {بررسی شیوع آنتی بادی پراکسیداز تیروئید و اثر درمانی لووتیروکسین بر پیامد بارداری در زنان باردار دارای آنتی بادی}, abstract_fa = {مقدمه: وجود آنتی بادی های ضد تیروئید منجر به عوارض اثبات شده ای مانند سقط جنین و افسردگی در طی بارداری و پس از آن می شود. ولی بررسی زیادی در مورد سایر مشکلات دوران بارداری از جمله زایمان زودرس، پارگی زودرس مامبران و هم چنین اثر آن بر میانگین وزن نوزاد و نمره آپگار نوزاد انجام نشده است. مطالعات نشان داده اند که درمان با لووتیروکسین ممکن است در پیشگیری از عوارض جنینی و مادری این عارضه مؤثر باشد. مطالعه حاضر با هدف تعیین شیوع آنتی‌ بادی پراکسیداز تیروئید و عوارض بارداری همراه با آن و بررسی تأثیر درمانی لووتیروکسین در زنان باردار دارای این آنتی ‌بادی ها در کاهش عوارض بارداری انجام شد. روش کار: این مطالعه کارآزمایی بالینی یک سوکور در سال 89-1388 بر روی 300 زن باردار مراجعه کننده‌ به مراکز بهداشتی سطح شهر سنندج که در سه ماهه‌ اول بارداری قرار داشتند، انجام شد. از 266 زن باردار بدون علائم بالینی هیپوتیروئیدی،32 زن باردار دارای آنتی بادی پراکسیداز تیروئید بودند که به دو گروه 16 نفره تقسیم شدند. گروه مداخله روزانه 50 میکروگرم لووتیروکسین در طی بارداری دریافت کردند. گروه کنترل تحت درمان خاصی قرار نگرفتند. پیامدهای بارداری افراد تا پایان بارداری بررسی و ثبت شد. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماریSPSS  (نسخه 5/11) و آزمون های کای دو، فیشر و کروسکال والیس انجام شد. میزان معناداریp  کمتر از 05/0، معنی دار در نظر گرفته شد. یافته ها: از 266 زن باردار، 32 نفر (12%) دارای آنتی بادی پراکسیداز تیروئید بودند. گروه های مورد مطالعه از نظر نوع زایمان، طول و مدت زایمان تفاوت آماری معنی داری نداشتند (0/37=p). اگرچه زایمان زودرس و پارگی زودرس مامبران در گروه درمان شده کمتر بود و نمره آپگار نوزاد و وزن نوزاد بیشتر از گروه شاهد بود، ولی این تفاوت از نظر آماری معنی دار نبود( 0/15=p ا34/0=p ا15/0=p). نتیجه گیری: اضافه کردن آزمایش آنتی بادی پراکسیداز تیروئید به مراقبت های دوران بارداری و درمان با لوتیروکسین در افراد دارای آنتی بادی پراکسیداز تیروئید می تواند در کاهش زایمان زودرس و پارگی زودرس مامبران و افزایش میانگین وزن نوزاد و نمره آپگار نوزاد موثر باشد.}, keywords_fa = {آنتی بادی پراکسیداز تیروئید,سقط,لووتیروکسین,پارگی زودرس کیسه آب}, url = {https://ijogi.mums.ac.ir/article_3248.html}, eprint = {https://ijogi.mums.ac.ir/article_3248_6ce77cb47b4e4a7093c2f04df0a22468.pdf} }
