As one of the causes of shoulder pain is referred pain caused by diaphragm irritation due to existence of blood and amniotic fluid in the peritoneal cavity after surgery, we decided to investigate the comparison between placing surgical sponges on both sides of uterus and reverse trendelenburg position of 30 degree to reduce the incidence of shoulder pain after cesarean section with spinal anesthesia. Methods: This three blind clinical trial was conducted on 201 women undergoing cesarean section in Niknafas hospital of Rafsanjan, Iran in 2011. Women were randomly divided into three groups. Surgeries were performed by two surgeons equally in each group. In group A, spinal anesthesia was performed and 2 surgical sponges with a radio opaque line impregnated with normal saline were placed on both sides of uterus and operation continued in supine position with no angle with horizon line. In group B, patients were operated in reverse trendelenburg position of 30 degree to horizon line without placing surgical sponges. In group C, cesarean was performed without any surgical sponges and in supine position. Severity of shoulder pain was assessed 24 hours after surgery in right, left or both shoulders in all patients. Data were analyzed using SPSS software (version 18) and one way ANOVA, chi-square, Post-hoc tests and t-test. P value less than 0.05 was considered significant. Results: Mean of pain severity had no significant differences between the three sites of pain (p=0.728). No significant differences were observed in pain severity between the three groups (p=0.111) and between the operations were performed by the two surgeons (p=0.430). Conclusion: According to no statistically significant difference between groups in pain severity, there might be another agent exclusive of blood and amniotic fluid that irritates diaphragm and cause shoulder pain.}, keywords = {Shoulder pain,Anesthesia/Spinal,Cesarean section}, title_fa = {مقایسه تأثیر قرار دادن لنگاز در طرفین رحم و وضعیت 30 درجه ترندلنبرگ معکوس در کاهش بروز درد شانه به دنبال عمل جراحی سزارین تحت بی حسی نخاعی}, abstract_fa = {مقدمه: درد شانه از جمله شکایات شایع زنان پس از عمل جراحی سزارین میباشد که گاهی اوقات بسیار شدید بوده و باعث اختلال عملکرد مادر در شیردهی و مراقبت از نوزاد میشود. یکی از دلایل درد شانه، درد ارجاعی ناشی از تحریک دیافراگم به دلیل وجود خون و مایع آمنیوتیک در حفره صفاق پس از جراحی میباشد، لذا مطالعه حاضر با هدف بررسی مقایسه ای تأثیر قرار دادن لنگاز در طرفین رحم و وضعیت 30 درجه ترندلنبرگ معکوس در کاهش بروز درد شانه به دنبال عمل جراحی سزارین تحت بی حسی نخاعی انجام شد. روش کار: این مطالعه کارآزمایی بالینی سه سوکور در سال 1391 بر روی 201 زن داوطلب سزارین در بیمارستان نیک نفس رفسنجان انجام شد. افراد به طور مساوی به سه گروه تقسیم شدند. جراحیها توسط دو جراح و به تعداد مساوی در هر گروه انجام شد. در بیماران گروه A، پس از انجام بی حسی نخاعی، دو عدد لنگاز آغشته به سرم نمکی با خط حاجب در طرفین رحم قرار داده شد و عمل جراحی در وضعیت خوابیده به پشت و در زاویه صفر نسبت به افق انجام شد. در گروه B، بیماران در حین عمل در وضعیت 30 درجه ترندلنبرگ معکوس و بدون استفاده از لنگاز قرار گرفتند. در بیماران گروه C، عمل سزارین بدون قرار دادن لنگاز و در وضعیت خوابیده به پشت انجام شد. شدت درد بیماران 24 ساعت پس از عمل در شانه راست، چپ یا هر دو شانه ارزیابی شد. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخۀ 18) و آزمون های آنالیز واریانس یک طرفه، کای دو، آزمون های تعقیبی PostHoc و آزمون تی تست دو نمونه ای انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد. یافته ها: اختلاف میانگین شدت درد بین سه محل درد معنی دار نبود (728/0=p). همچنین اختلاف معنی داری بین میانگین شدت درد در بین سه گروه (111/0=p) و بین جراحی های انجام شده توسط جراح A و B مشاهده نشد (430/0=p). نتیجه گیری: با توجه به عدم وجود اختلاف معنی دار در گروه شاهد و گروه استفاده از لنگاز و ترندلنبرگ معکوس، احتمالاً عامل دیگری غیر از وجود خون و مایع آمنیوتیک، باعث تحریک دیافراگم و درد ارجاعی شانه میشود.}, keywords_fa = {بی حسی نخاعی,درد شانه,سزارین}, url = {https://ijogi.mums.ac.ir/article_2854.html}, eprint = {https://ijogi.mums.ac.ir/article_2854_130b5d93d8e27193f1393165e40c5304.pdf} }
