Comparison of marital satisfaction in fertile and infertile couples and its relationship with sexual problems

Introduction: Infertility as a crisis in couples' life, not only creates psychological problems, but also, it can act as a powerful impact on the relationships between couples. In many cases, infertility affects a great impact on couples' sexual relationships. According to different results of studies about the effect of infertility on marital and sexual relationships, this study was performed with aim to compare marital satisfaction in fertile and infertile couples and evaluation of its relationship with sexual problems. Methods: This cross-sectional study was performed on 110 infertile and 110 fertile couples referred to Montaserieh Infertility Center and five Health-Medical centers in Mashhad in 2013. Data collection tools included personal characteristic questionnaire, Nathan H. Azrin marital satisfaction and Glombok-Rust questionnaire. Data were analyzed using SPSS software (version 16) and Mann-Whitney, Chi-square, Kruskal-Wallis, Pearson and Spearman correlation coefficient tests. PResults: The mean score of marital satisfaction in fertile and infertile women was 64.4±12.9, 65.2±12.5, respectively that was in moderate level in both groups. According to the results of Mann-Whitney test, there was no significant difference in the marital satisfaction score in fertile and infertile couples (P=0.45). There was a significant inverse correlation between marital satisfaction with sexual problems in both groups (r=-0.35, p<0.0001). Conclusion: Regarding the relationship between marital satisfaction and sexual problems, it seems that use of the approaches to identify and treat sexual problems is required in health and infertility centers to increase marital satisfaction.

Introduction: Diabetes is the most common medical complication in pregnancy. Increased levels of inflammatory cytokines among women with gestational diabetes is suggestive of inflammation and infection as one of the etiologies of gestational diabetes. These cytokines can enter from periodontal tissues into the mother's bloodstream and severely stimulate maternal systemic inflammatory response. Various risk factors have been mentioned for gestational diabetes, but just a few studies have assessed the relationship between periodontal disease and gestational diabetes. This study was performed with aim to evaluate the relationship between periodontal disease and gestational diabetes. Methods: this historical cohort study was conducted on 91 women affected to periodontal disease (case group) and 186 healthy women (control group) referred to selected health-care centers of Mashhad in 2014. Data gathering method included questionnaire, observation checklist, dental probe for assessment of periodontal patients and spectrophotometry for determination of blood sugar. Data was analyzed by SPSS software (version 16), t-test, Mann-Whitney, Chi-square, and Logistic regression. PResults: Affecting to periodontal disease was related to gestational diabetes (P<0.05). Affecting risk ratio to diabetes in women with periodontal disease was 5.7 times more than people without periodontal disease. Conclusion: Affecting to periodontal disease (disseminated chronic priodontitis) can be a risk factor for affecting to gestational diabetes.

A rare case of a pelvic mass protest

Introduction: uretero pelvic junction obstruction (UPJO) is partial or complete obstruction of urine flow that occurs at the site where the ureter enters to the renal pelvis. UPJO occurs in 22-37% of ectopic kidneys. In pelvic ectopic kidney in young adults, UPJO protests with clinical symptoms of intermittent pain in the side or abdomen that may be similar to the symptoms of pelvic mass. The aim of this report is to introduce a rare case of a pelvic mass protest. Case Report: A 18-year-old virgin woman referred with complain of menstrual irregularities that has performed ultrasound one last year and diagnosed with an ovarian cyst at ultrasound. Early ultrasound reported a cyst with size of about 2 × 12 cm in right ovary that after 3 months consumption of contraceptive pills, a cyst 98 × 95 × 68 mm with possible origin of right adnexal was reported. Due to abdominal pain and large size of cyst, the patient was candidate for laparotomy. The origin of cyst was diagnosed as the renal pelvis. The right kidney was ectopic and contained UPJO. Pyeloplasty was conducted by Anderson Hys method and Double-J Stent was inserted. Conclusion: In the cases of pelvic mass, it is very important to consider the possible non-gynecologic origin of the mass, especially when there is no possibility of a full gynecological examination because the patient was Virgin. All differential diagnosis of pelvic masses, especially with urologic and gastrointestinal origin should be considered and diagnostic procedures should be performed before the surgery.
