فراوانی و علل قصور مامایی در پرونده های ارجاعی به اداره کل پزشکی قانونی اصفهان طی سال های 1384 تا 1388

چکیده

مقدمه: ماماها همواره و از دیرباز، اولین متصدیان امر مراقبت از مادر و جنین شناخته شده اند. همین امر باعث فراهم شدن زمینه بروز خطا های جبران ناپذیر در این حرفه شده است. مطالعه حاضر با هدف بررسی فراوانی و علل قصور مامایی در پرونده های ارجاعی به اداره کل پزشکی قانونی اصفهان طی سال های 1384 تا 1388 انجام شد.
روش کار: این مطالعه توصیفی و مقطعی بر روی تمامی شکایات ارجاعی به دفتر کمیسیون پزشکی اداره کل پزشکی قانونی استان اصفهان با موضوع شکایت از کادر مامایی شاغل در بیمارستان ها، مطب و سایر مراکز درمانی طی سال های 1384 تا 1388انجام شد. ابزار گردآوری داده ها چک لیست بود. روش جمع آوری داده ها بررسی منابع و مشاهده بود. داده ها با استفاده از نرم افزار آماری SPSS (نسخه 11) مورد تجزیه و تحلیل قرار گرفتند.
یافته ها: از 708 مورد شکایت مطرح شده، 41 مورد (8/5%) آن مربوط به ماماها بود. از 41 نفری که از آن ها شکایت شده بود، 18 نفر (9/43%) از ماماها مرتکب قصور شده بودند. ماماها در 9/38 موارد به دلیل بی مبالاتی و در 4/44 موارد به دلیل عدم رعایت نظامات دولتی مقصر شناخته شده بودند. 13 نفر (7/31%) از ماماهایی که از آن ها شکایت شده بود در رده سنی 35 تا 40 سال قرار داشتند که بیشترین تعداد را به خود اختصاص دادند. 35 نفر (4/85%) از ماماهایی که از آن ها شکایت شده بود در بیمارستان ها و 6 نفر (2/12%) از آن ها در خانه های بهداشت به بیماران ارائه خدمت کرده بودند.
نتیجه گیری: با توجه به اهمیت حرفه مامایی، شاغلین این حرفه باید در مورد قوانین و مقررات پزشکی، جرائم و تخلف، قانون دیات، مسائل سقط جنین و موارد دیگر اطلاع کافی داشته باشند.

چکیده [English]

Introduction: Midwives continually are known as the first custodians of mothers and fetus care. This item causes midwifes problems in their work and irreparable faults. This study investigated the frequency and reasons of midwifes malpractices in cases referred to general office of legal medicine of Isfahan, Iran during 2005-2009.
Methods: This descriptive-cross sectional study was conducted on all cases referred to general office of legal medicine of Isfahan, Iran during 2005-2009 with claims from midwifery staffs occupied in hospitals, clinics and other healthcare centers. Data were gathered by a checklist. Data collection methods were resource review and observation. Data were analyzed by SPSS software (version 11).
Results: 41 claims (5.8%) of 708 claims were from midwifes. 18 midwives from 41 (43.9%) were convicted. Negligence (38.9%) and carelessness of governmental rules (44.4%) were the main reasons of midwives malpractices. 13 midwives (31.7%) allocated the most frequency of claims. They were in the age range of 35 to 65 years. 35 midwives (85.4%) of 41 gave services in hospitals and 6 cases (12.2%) in home-health.
Conclusion: With attention to importance of midwifery, the practitioners of this occupation should be informed about medical laws and regulations, crimes and infractions, blood money law, abortion laws and other legal materials.
